单切口双钢板固定治疗胫腓骨远端骨折的效果观察

目的分析单切口双钢板固定治疗胫腓骨远端骨折的临床效果及体会。方法回顾性分析21例胫腓骨远端骨折患者的临床资料，采用单切口双钢板固定治疗方法，观察治疗效果。结果所有随访9个月，Johner-wruh评价标准对所有患者的术后功能恢复情况进行评价，21例患者中，达到优级标准者6例，达到良级标准者11例，达到中级标准者2例，余者2例功能恢复较差，本组21例患者术后功能恢复优良率为80．95％。术后发生足背外侧麻木、皮肤浅表坏死等各1例；未发生深部感染、骨髓炎等严重并发症。结论单切口双钢板固定治疗胫腓骨远端骨折可清晰暴露胫骨及腓骨远端，同时进行钢板内固定，可更好保护骨折处软组织，降低伤口出现术后并发症的机率，有利于患者关节功能的恢复，故值得临床推广。

Objective to analyze the clinical effect and experience of single incision double plate fixation in the treatment of distal tibial fractures. Methods the clinical data of 21 cases of distal tibia and fibula fractures were retrospectively analyzed. The clinical data of patients with distal tibia and fibula fractures were treated by single incision double plate fixation. Results all patients were followed up for 9 months, the evaluation criterion of Johner-wruh on the function of all patients with postoperative recovery were evaluated in 21 patients, 6 cases reached excellent standard, 11 cases achieved good grade standard, 2 cases reach the intermediate standard, more than 2 cases of poor functional recovery, 21 cases of this group of patients after the function recovery rate was 80.95%. Occurrence of lateral dorsum of foot numbness, superficial skin necrosis in 1 cases after operation; no serious complications occurred in deep infection, osteomyelitis. Conclusion single incision double plate fixation for distal tibia and fibula fracture can be clearly exposed tibia and fibula distal, and at the same time, can better protect the soft tissue, reduce the risk of postoperative complications, is conducive to the recovery of joint function, so it is worth clinical promotion.
