中医综合疗法治疗初期糖尿病足临床观察

Clinical effect of integrated traditional Chinese medicine therapy in treatment of early-stage diabetic foot

目的:观察中医综合疗法治疗初期糖尿病足的临床疗效。方法:将80例初期糖尿病足患者随机分为治疗组与对照组各40例。在常规降糖治疗的基础上,治疗组采用口服脉络宁口服液、针刺、外用药熏洗等中医综合疗法治疗,对照组口服西洛他唑治疗。治疗3个月后比较两组血管狭窄度分级、踝肱指数、浅感觉检查、皮肤状况、阴虚血瘀症状数量等初期糖尿病足高危因素的变化。结果:两组各高危因素治疗前后组内比较,差异均有统计学意义(P<0.05);治疗后阴虚血瘀症状数量组间比较,差异有统计学意义(P<0.01),其余高危因素组间比较,差异无统计学意义(P>0.05)。结论:中医综合治疗与口服西洛他唑治疗均有预防初期糖尿病足加重的作用,但中医综合治疗改善糖尿病患者阴虚血瘀症状的作用更显著。

Objective： To investigate the clinical effect of integrated traditional Chinese medicine therapy in the treatment of early- stage diabetic foot. Methods： A total of 80 patients with early- stage diabetic foot were randomly divided into treatment group and control group,with 40 patients in each group. In addition to the conventional hypoglycemic therapy,the patients in the treatment group were given the integrated traditional Chinese medicine therapy including Mailuoning oral liquid,acupuncture,and fumigation- washing with drugs for external application,and those in the control group were given oral cilostazol. The changes in high- risk factors for early- stage diabetic foot including vascular stenosisdegree,ankle- brachial index,superficial sensation examination,skin condition,and number of yin- deficiency and blood- stasis symptoms were compared between the two groups after 3 months of treatment. Results： There were significant changes in high- risk factors after treatment in both groups（ P〈0. 05）. There was a significant difference in the number of yin- deficiency and blood- stasis symptoms after treatment between the two groups（ P〈0. 01）,but there were no significant differences in the other high- risk factors between the two groups（ P〈0. 05）. Conclusion： Both integrated traditional Chinese medicine therapy and oral cilostazol can prevent the aggravation of early- stage diabetic foot,but integrated traditional Chinese medicine therapy has a significantly better effect in improving the symptoms of yin- deficiency and blood- stasis.
