摘要
目的:探讨有机磷中毒采用血液灌流(HP)联合乌司他丁治疗的临床效果及对患者血清细胞因子的影响。方法:选取2012-03-2016-06月恩施州民族医院收治的100例有机磷农药中毒患者,根据就诊单双号分为HP组47例、联合组53例,2组均采用HP结合常规疗法,联合组同时给予乌司他丁治疗,对比2组的治疗效果。结果:联合组的阿托品用量、昏迷时间、住院时间均低于HP组,差异具有统计学意义(P<0.05);治疗前、治疗1d以后,2组患者的血清TNF-α、IL-6、抵抗素、内脂素水平差异无统计学意义;治疗5d后,联合组的血清TNF-α、IL-6、抵抗素、内脂素水平均低于HP组(P<0.05);治疗前、治疗1d以后,2组患者的CHE、PaO_2、PaCO_2差异无统计学意义;治疗5d后,联合组患者的CHE、PaO_2水平高于HP组(P<0.05)。联合组的治愈率94.34%高于HP组的74.47%(P<0.05)。结论:有机磷中毒采用HP联合乌司他丁治疗的临床效果优于单纯HP治疗。
Objective:To investigate the blood perfusion of organophosphorus poisoning(HP)combined with ulinastatin treatment and clinical effect of statins on serum cytokines in patients.Method:According to the patients divided into the treatment group HP 47 cases,53cases of the combined group of 100 cases of organophosphorus pesticide from March 2012 to June 2016,Enshi National Hospital poisoning,two groups were treated with HP combined with conventional therapy,combined group was given Ulinastatin treatment,compared two groups of treatment effect.Result:The combination group dosage of atropine,coma time and hospitalization time were lower than the HP group,the difference was statistically significant(P〈0.05);after 1days of treatment,before treatment,two groups of patients with serum TNF-,IL-6,resistin,visfatin level difference was not statistically significant;after 5days of treatment,the combination group serum TNF-,IL-6,resistin,visfatin levels were lower than those in group HP(P〈0.05);after 1days of treatment,before treatment,two groups of patients with CHE,PaO2,PaCO2,the difference was not statistically significant;after 5days of treatment,PaO2 levels of group CHE,higher than that of group HP(P〈0.05).The cure rate of the combined group was 74.47% higher than that of the group HP 94.34%(P〈0.05).Conclusion:Using HP combined with ulinastatin in the treatment of the clinical effect is better than pure HP treatment of organophosphorus poisoning.
