北京地区11家医疗机构处方点评工作开展状况调研与分析

Investigation and analysis of prescription comment working status in eleven medical institutions of Beijing

目的:了解北京地区处方点评工作开展现状和《医院处方点评管理规范(试行)》相关规定落实情况。方法:通过问卷调查的形式,对北京地区11家医疗机构的11名药学人员进行调研,了解各单位处方点评开展方式、点评结果利用方法、对处方点评的评价、点评中存在的问题等方面内容。结果:11家医疗机构中,每月开展1次点评的为11家(占100%);安排主管药师参与点评的为11家(占100%);已成立处方点评专家组的为4家(占36.4%);已开展专项处方点评工作的为9家(占81.8%);将点评结果报送医院行政管理部门的为9家(占81.8%);"医院医疗管理部门对点评结果进行审核并定期公布,通报不合理处方"的为9家(占81.8%);认为"本单位处方点评工作的开展在改进处方质量方面,取得一定成效,不合理处方率逐步降低"的为9家(占81.8%);点评人员、临床参与度等方面问题是开展处方点评过程中存在的最大问题。结论:医疗机构应进一步加强对《医院处方点评管理规范(试行)》的落实和执行,充分发挥处方点评专家组、行政干预在处方点评中的作用。

Objective： To investigate the working status of prescription comment and the implementation degree of Management Practices of Hospital Prescription Comment（trial） in Beijing. Methods： Eleven pharmacists from eleven hospitals in Beijing were questioned via survey investigation tables, in order to learn the way to conduct prescription comment, the method to use prescription comment results, the view to evaluate prescription comment, and the troubles faced during prescription comment in these eleven hospitals. Results： Among the eleven hospitals, all of them（100%） implemented prescription comment every month, and arranged pharmacists-incharge to participate in prescription comment. Four of them（36.4%） had established prescription comment expert group. Nine of them（81.8%） had operated special prescription comment, and submitted prescription comment results to hospital administration department. Nine of them（81.8%） checked, published and communicated the prescription comment results. In additions, nine of them（81.8%） considered that the execution of prescription comment had improved prescription quality, and reduced the ratio of irrational prescriptions. However, some problems, including the quality of prescription commenters and the participation degree of clinical staff, were deeply influencing the execution of prescription comment. Conclusion： The implementation of Management Practices of Hospital Prescription Comment（trial） should be more powerful in medical institutions, in order to make the effect of prescription comment experts group and the effect of administrative intervention to be fully used.
