卒中后超早期活动结合针刺治疗急性缺血性卒中临床研究

Clinical Research of Ultra-early Activities Combined with Acupuncture in Treating Acute Ischemic Stroke

目的:观察卒中后24小时内超早期、密集下床活动结合醒脑开窍针刺法治疗急性缺血性卒中患者的行走功能恢复和日常生活活动改善的疗效。方法 :将61例急性缺血性卒中患者随机分为2组,对照组接受卒中单元治疗,治疗组在对照组基础上采用超早期密集下床活动和醒脑开窍法针刺治疗,观察卒中患者独立行走50米所需中位天数及治疗前、治疗7天、14天、3月末的Barthel指数和治疗前、治疗14天、3月末的肢体运动能力Fugl-Meyer评分。结果:2组至能够行走50米所需的中位天数比较显示治疗组比对照组更快的恢复独立行走。2组生活质量Barthel指数评分和肢体运动能力Fugl-Meyer评分较治疗前增加(P<0.05),差异有显著性意义,治疗组明显优于对照组。结论:超早期和更密集的下床活动结合针刺治疗可能是缺血性脑卒中患者恢复独立行走的快速方法,并可能改善日常生活功能结局。

Objective： To observe the therapeutic effect of ultra-early and intensive out-of-bed activities combined with Xingnao Kaiqiao Acupuncture on recovery of walking function and improvement of daily living activities in patients with acute ischemic stroke within 24 hours after stroke. Methods： Sixty-one patients with acute ischemic stroke were randomly divided into two groups. The control group was treated with stroke unit. The treatment group was treated with ultraearly intensive crowding out of bed and acupuncture. The Barthel index was observed before, 7 days, 14 days and 3 months after treatment. The Fugl-Meyer score of limb motor ability before treatment, 14 days and 3 months was observed. Results： The comparison of the median days needed to walk 50 meters between the two groups showed that the treatment group recovered more independently than the control group. The scores of Barthel index and Fugl-Meyer score of quality of life of the two groups were significantly higher than those of the control group（P 0.05）, and the difference was statistically significant. Conclusion： Ultra-early and more intensive out-of-bed activities combined with acupuncture may be a rapid method to restore independent walking in patients with ischemic stroke and may improve the outcome of daily life.
