基于新安医学“固本培元”思想治疗癌症恶液质临床研究

Research of treating cancer cachexia based on“Guben Peiyuan” thought of xin'an medcine

目的:癌症患者晚期出现恶液质极为常见,表现为代谢紊乱为主的一系列虚弱症状。采用新安医学"固本培元"思想重视后天之本,同时兼顾先天之本,固护人体元气来治疗癌症恶液质来观察其疗效。方法:将符合纳入标准的60例癌症恶液质患者随机分为2组,A组(中药治疗组)予以营养支持治疗的同时行中药2次/d,早晚饭后30min服用,共治疗14d。B组(对症治疗组)予以营养支持治疗,治疗14d。结果:治疗后中药治疗组血清白蛋白及前白蛋白较对症治疗组升高(P<0.05),差异有统计学意义。中药治疗组及对症治疗组在治疗癌症恶液质消瘦、气短、乏力、纳差、腹胀5个方面均具有降低症状积分的作用。治疗后,组间比较,中药治疗组较对症治疗组在消瘦(P=0.048,P<0.05)、气短(P=0.038,P<0.05)、乏力(P=0.007,P<0.05)、纳差(P=0.040,P<0.05)4个方面具有改善作用,而在腹胀方面中药治疗组较对症治疗组相比无统计学意义(P=0.120,P>0.05)。结论:新安医学"固本培元"思想指导下采用益气健脾、补肾益精功效的中药治疗癌症恶液质具有明显的疗效,可改善患者生活质量。

Objective：Cachexia is extremely common in advanced cancer patients,which is mainly characterized by metabolic disorder and a series of weak symptoms. We use ＂ Guben Peiyuan ＂ thought of xin＇an medicine to treat cancer cachexia and observe its effect. Methods：60 patients with cancer cachexia were randomly divided into 2 groups：Group A（traditional Chinese medicine treatment group）. The patients in Group A were treated by nutritional support treatment and traditional Chinese medicine twice A day at the same time,sooner or later take 30 minutes after A meal,A total of 14 days treatment. Group B（nutritional support treatment group）The patients in Group B were treated only by nutritional support treatment. Results：The level of serum albumin and prealbumin in Group A were more higher than the level in the Group B（P0.05）,the difference was statistically significant. Traditional Chinese medicine treatment group and nutritional support treatment group both have the effect of reducing symptoms integral in the treatment of cancer Cachexia.,which is manifested as weight loss,shortness of breath,fatigue,anorexia,abdominal distension. After treatment,the comparison between groups,Chinese medicine treatment group compared with nutritional support treatment group in weight loss（P= 0.048,P0.048）,shortness of breath（P=0.038,P0.038）,fatigue（P=0.007,P0.05）,anorexia（P= 0.040,P0.05）improved the effects of four aspects,and Chinese medicine treatment group in terms of abdominal distension has no statistical significance compared with nutritional support treatment group（P=0.120,P0.05）. Conclusion：The traditional Chinese medicine which is under the guidance of Guben Peiyuan＂thought of xin＇an medcine can improve the patients quality of life by replenishing Qi to invigorate spleen and nourishing kidney and enriching essence.
