摘要 目的 探讨血栓弹力图仪(TEG)在行急诊冠状动脉旁路移植术(ECABG)中的应用价值.方法 回顾分析2014年3月至2016年4月期间在我院心脏外科行ECABG的46例患者.在本院或外院行冠脉造影术后,因病情危重,急诊转我院心脏外科拟行ECABG,术前行血栓弹力图检查,评价血小板及凝血情况,决定术前备血,指导术中及术后用血.结果 46例患者术前有针对性备血,及时补充血小板及相关血制品,手术平稳过度,术后早期出血3例.结论 经过术前血栓弹力图检测,能明确患者血小板功能、基本血凝状况,根据情况有的放矢,准备相应血制品及实施相应对策,从而提高手术质量,也避免不必要的血制品浪费. Objective Thrombus elastic graph instrument (TEG) in emergency coronary artery bypass grafting (ECABG) application value. Methods Retrospective analysis was carried in 56 patients undergoing E- CABG in our hospital from March 2014 to April 2016.Because of critical coronary lesions, the patients admitted to our hospital and accepted ECABG. TEG was measured preoperative for evaluation of platelets and clotting function, and deciding the preoperative preparation of blood, guiding intraoperative and postoperative transfusion. Results 46 cases of patients accepted timely supplement of platelets and related blood products and the operation was smooth, 3 cases with early postoperative bleeding. Conclusion After preoperative blood clots elastic figure test, platelet function in patients with clear, the basic conditions of coagulation, according to the situation, target, pre- pare the corresponding blood products and implement corresponding of operation, and avoid unnecessary blood product waste.
