摘要
目的探讨有创与无创序贯性机械通气治疗慢性阻塞性肺疾病(COPD)合并呼吸衰竭的临床疗效。方法 72例COPD合并呼吸衰竭患者,按照治疗方式分为治疗组和对照组,各36例。治疗组采用有创与无创序贯性机械通气治疗,对照组采用单纯有创机械通气治疗,比较两组治疗效果以及住院时间。结果治疗组患者显效25例,有效10例,无效1例,总有效率为97.2%;对照组患者显效14例,有效15例,无效7例,总有效率为80.6%;两组总有效率比较差异有统计学意义(χ~2=5.063,P=0.024〈0.05)。治疗组患者平均住院时间为(14.2±2.1)d,短于对照组患者的(19.7±3.4)d,差异有统计学意义(t=8.258,P=0.000〈0.05)。结论有创与无创序贯性机械通气治疗COPD合并呼吸衰竭的临床疗效肯定,且住院时间较短,临床可推广。
Objective To investigate clinical effect by invasive and noninvasive sequential mechanical ventilation in the treatment of chronic obstructive pulmonary disease(COPD)complicated with respiratory failure. Methods A total of 72 patients of COPD complicated with respiratory failure were divided by treatment measures into treatment group and control group, with 36 cases in each group. The treatment group received invasive and noninvasive sequential mechanical ventilation for treatment, and the control group received invasive sequential mechanical ventilation alone for treatment. Comparison was made on curative effect and hospital stay time between the two groups. Results The treatment group had 25 excellent cases, 10 effective cases and 1 ineffective case, with total effective rate as 97.2%. The control group had 14 excellent cases, 15 effective cases and 7 ineffective cases, with total effective rate as 80.6%. The difference of total effective rate had statistical significance between the two groups(χ~2=5.063, P=0.0240.05). The treatment group had mean hospital stay time as(14.2±2.1)d, which was shorter than(19.7±3.4)d in the control group, and their difference had statistical significance(t=8.258, P=0.0000.05). Conclusion Invasive and noninvasive sequential mechanical ventilation provides affirmative clinical effect in treating COPD complicated with respiratory failure, along with short hospital stay time. It is worth clinical promotion.
