探讨PICC置管在极低出生体重儿肠外营养中的应用效果

Investigation of application effect by PICC catheterization in parenteral nutrition for very low birth weight infants

目的分析经外周静脉置入中心静脉导管(PICC)置管在极低出生体重儿肠外营养中的应用效果。方法 100例体重<1500 g的极低出生体重儿,按照随机方式分成对照组和观察组,各50例。对照组采用外周留置针输液,观察组采用PICC输液。对比两组效果。结果对照组患儿的体重增长为(705.2±6.8)g,静脉输液时间为(20.6±2.2)h,住院时间为(53.3±13.8)d;观察组患儿的体重增长为(733.4±12.7)g,静脉输液时间为(27.2±2.8)h,住院时间为(42.1±15.4)d;观察组在体重增长情况、静脉输液时间以及住院时间方面均显著优于对照组,差异有统计学意义(P<0.05)。观察组感染发生率2.0%(1/50)显著低于对照组的14.0%(7/50),差异有统计学意义(P<0.05)。结论在极低出生体重儿肠外营养中应用PICC置管能保证营养液正常供给,促进患儿体格发育,降低感染发生率,缩短静脉输液时间和住院时间,具有临床应用价值。

Objective To analyze application effect by peripherally inserted central catheter（PICC）catheterization in parenteral nutrition for very low birth weight infants. Methods A total of 100 very low birth weight infants 1500 g were randomly divided into control group and observation group, with 50 cases in each group. The control group received peripheral indwelling needle infusion, and the observation group received PICC infusion. Effects were compared between the two groups. Results The control group had ponderal growth as（705.2±6.8）g, intravenous infusion time as（20.6±2.2）h and hospital stay time as（53.3±13.8）d. The observation group had ponderal growth as（733.4±12.7）g, intravenous infusion time as（27.2±2.8）h and hospital stay time as（42.1±15.4）d. The observation group had all better ponderal growth, intravenous infusion time and hospital stay time than the control group, and their difference had statistical significance（P〈0.05）. The observation group had obviously lower incidence of infection as 2.0%（1/50）than 14.0%（7/50）in the control group, and the difference had statistical significance（P〈0.05）. Conclusion Implement of PICC catheterization in parenteral nutrition for very low birth weight infants can ensure nutrient solution supply, accelerate physical development, reduce incidence of infection, and shorten intravenous infusion time and hospital stay time. This method contains clinical application value.
