定位旋转手法结合电针治疗椎动脉型颈椎病临床研究

Clinical Study of Vertebroarterial Cervical Spondylopathy Treated by Positioning Spinning Technique Combined with Electro-Acupuncture

目的：探讨定位旋转手法结合电针治疗椎动脉型颈椎病的临床疗效。方法：90例确诊患者随机分为观察组及对照组各45例。观察组运用定点旋转的推拿手法结合针刺,再接G6805-Ⅰ型电针,疏密波治疗。对照组仅采取单纯针刺加电针治疗。两组每次均治疗30 min,6次为1疗程,疗程间休息2~3 d,3个疗程后评价疗效。并于每组随机抽取10例患者,观察治疗前后彩色多普勒经颅检测治疗前后颅外段左、右椎动脉最窄部位血管内径及收缩期最大血流速度。结果：治疗后两组左、右侧椎动脉最狭窄部位血管内径和收缩期最大血流速度均有不同程度的改善,观察组的改善效应更为明显,均优于对照组（均P〈0.05）。两组的治愈率分别为84.44%和20.00%,总有效率分别为100.00%和77.78%,观察组均显著优于对照组（均P〈0.01）。结论：定位旋转手法结合电针治疗椎动脉型颈椎病的临床疗效明显,其对左、右椎动脉最狭窄部位血管内径及收缩期最大血流速度的改善作用均明显优于单纯电针治疗。

Objective：To discuss the clinical curative effects of vertebroarterial cervical spondylopathy treated by positioning spinning technique combined with electro-acupuncture. Methods：Ninety patients were randomly divided into observation group and control group,with 45 patients in each group. The observation group was given fixed-point rotation massage combined with electro-acupuncture. The electro-acupuncture apparatus is G6805-I,and dilatational wave was applied. The control group was given acupuncture plus electro-acupuncture. Both groups were treated for 30 minutes,and 6 times were a therapeutic course. The treatments lasted for 3 courses,and patients rested 2 or 3 days between each course. After the treatment,the curative effects were evaluated. Ten patients were randomly selected from each group and were examined with color Doppler ultrasonography to detect the thinnest vascular diameter of the left and right extracranial vertebral artery（ECVA） and the maximum systolic blood flow velocity before and after treatment. Results：After treatment,the thinnest vascular diameter of the left and right ECVA and systolic blood flow velocity were improved.The observation group were significantly better than the control group（P〈0.05）. The curative rates of the two groups were 84.44% and 20.00% respectively,and the total effective rates were 100.00% and 77.78% respectively. The observation group were significantly better than the control group（P〈0.01）. Conclusion：Positioning spinning technique combined with electro-acupuncture treatment for vertebral artery type of cervical spondylosis showed significant clinical efficacy,and this treatment was obviously better than acupuncture on the improvement of the thinnest vascular diameter of the left and right ECVA and systolic blood flow velocity.
