血浆同型半胱氨酸水平与老年痴呆病情程度的相关性研究

Correlation Between Plasma Homocysteine Level and the Severity of Alzheimer's Disease

目的研究老年痴呆患者病情严重程度与血浆同型半胱氨酸（Hcy）水平的相关性,并探讨其临床上的意义。方法选取2014年7月~2015年6月收治的老年痴呆患者123例,将其作为研究组,另取同期的健康者41例作为对照组,采用循环酶法检测两组人员的血浆同型半胱氨酸的水平,并对比分析,依据患者痴呆严重程度,可将研究组患者分为重度、中度以及轻度患者;依据Hachinski缺血指数可将研究组患者分为血管性痴呆（VD）、混合性痴呆（MD）以及阿尔茨海默病（AD）。结果研究组中VD、MD、AD各组患者Hcy水平为（26.33±5.44）、（24.72±5.98）、（25.69±7.57）均明显比正常对照组（14.56±6.96）要高,差异显著（P〈0.05）;不同痴呆程度的患者,其血浆Hcy水平为（31.46±4.36）、（25.51±5.15）、（21.97±4.98）相互间比较,差异显著（P〈0.05）。随着患者病情严重程度的增加,其Hcy水平逐渐升高,患者认知功能越差。结论血浆中同型半胱氨酸含量越高,患者认知功能越差,高Hcy水平可能是老年痴呆发病重要危险因素,正相关于痴呆病情严重程度。临床上需重视老年痴呆患者Hcy水平变化。

Objective Patients with senile dementia severity and plasma homocysteine（Hcy） level of patients,and to explore its clinical significance.Methods 123 patients with senile dementia from July 2014 to June 2015 in our hospital were taken as the study group,another 41 healthy cases as the same the control group,measured by enzymatic cycling assay of the two groups of plasma homocysteine levels,and compared according to the patient＇s dementia severity,the study group patients were divided into severe,moderate and mild patients; according to the Hachinski index can be ischemia patients in study group were divided into vascular dementia（VD）,mixed dementia（MD） and Alzheimer＇s disease（AD）.Results the study group in VD,MD,Hcy levels in patients with AD were（26.33 ± 5.44）,（24.72± 5.98）,（25.69 ± 7.57） were higher than the normal control group（14.56 ± 6.96） to high（P〈0.05）,significant difference; different degree of dementia patients,the plasma level of Hcy for（31.46 ± 4.36）,（25.51 ± 5.15）,（21.97 ± 4.98） between,significant difference（P〈0.05）.With increase of the severity of the disease,the Hcy levels gradually increased,the worse cognitive function in patients with.Conclusions higher plasma homocysteine levels,cognitive function in patients with worse,high Hcy level may is one of the important risk factors of Alzheimer＇s disease,positive about the severity of dementia.Clinical need pay attention to Hcy level change in senile dementia patients.
