布托啡诺、吗啡用于高龄患者术后硬膜外自控镇痛的安全性比较

Comparison of the Safety of Butorphanol and Morphine in Postoperative Patient-Controlled Epidural Analgesia in Elderly Patients

目的观察布托啡诺、吗啡用于高龄患者术后硬膜外自控镇痛的安全性比较。方法60例高龄下肢手术患者行连续硬膜外麻醉随机分为两组:B组和M组。分别于术后2 h(T1)、6 h(T2)、12 h(T3)、24 h(T4)、48 h(T5)监测并记录患者潮气量(VT)、肺活量(VC)、呼吸频率(RR);抽取动脉血监测PH、Pa CO2、Pa O2、Sa O2等指标。结果 M组在T2、T3时RR、VC、Pa O2明显低于B组,且与同组T0时相比也明显降低有显著性差异(P<0.05);而Pa CO2在T2、T3时明显高于B组,且与同组T0时相比也明显升高,均有显著性差异(P<0.05)。结论布托啡诺应用于高龄患者硬膜外自控镇痛时,对呼吸系统抑制较轻,安全性好。

Objective To observe Bhutto butorphanol,morphine for elderly patients with postoperative epidural analgesia safety.Methods 60 cases of elderly patients with lower extremity surgery were randomly divided into two groups：group B and group M.Respectively after operation（T1） 2 h,6 h（T2）,12 h（T3）,24 h（T4）,and H（T5） monitoring and recording with moisture amount（VT）,vital capacity（VC）,respiratory rate（RR）; draw the arterial monitoring of blood p H,Pa CO2,Pa O2 and Sa O2 of indicators.Results in group M at T2 and T3 RR,VC,Pa O2 was significantly lower than that in group B and also significantly decreased significantly（P〈0.05） compared with the same group of t0; and Pa CO2 in T2,T3 was significantly higher than that of group B and was significantly increased,compared with the same group of t0.There were significant differences（P〈0.05）.Conclusions Bhutto butorphanol applied in elderly patients controlled epidural analgesia on respiratory inhibition,lighter,good safety.
