不同时间治疗妊娠期糖尿病对母婴预后的影响

Effect of the treatment of gestational diabetes mellitus under different treatment time on maternal and infant prognosis

目的探讨妊娠期糖尿病患者不同治疗时间对分娩方式、围生期母婴并发症的影响，为临床合理选择时机治疗该病提供理论参考。方法选择2009年3月至2015年2月住院分娩的86例妊娠期糖尿病患者为研究对象，根据确诊治疗时间分为中期组和后期组，中期组55例为妊娠中期确诊治疗的患者，后期组31例为孕34周后确诊治疗的患者，比较两组患者分娩方式、围生期孕产妇并发症发生情况和新生儿并发症发生情况。结果中期组剖宫产率为29.09%，明显低于后期组（77.42%），差异有统计学意义（P〈0.05）；中期组围生期孕妇各种并发症发生率明显低于后期组，差异有统计学意义（P〈0.05）；中期治疗组新生儿各种并发症及死亡情况明显低于后期组，差异有统计学意义（P〈0.05）。结论妊娠期糖尿病患者早期确诊、及时治疗可以降低剖宫产率，减少围生期母婴并发症的发生率，改善新生儿预后。

ObjectiveTo investigate the effects of different treatment time on delivery ways, the maternal and infant complications in the perinatal period of patients with gestational diabetes mellitus, in order to provide theoretical reference for clinical rational selection time to treat the disease.MethodsFrom March 2009 to February 2015, 86 cases of gestational diabetes mellitus hospitalized were selected as the research objects. According to the diagnosis treatment time, the patients were divided into middle group and late group. The 55 cases in the medium group were treated at mid pregnancy, and the 31 cases in the late group were treated at 34 weeks after pregnancy. The way of delivery, maternal complications occurred in the perinatal period and neonatal complications were compared.ResultsThe cesarean delivery rate was 29.09% in the middle group, which was significantly less than that of the late group （77.42%）, the difference was significant （P〈0.05）. The incidence of maternal complications occurred in the perinatal period in middle group was lower than that in late group, the difference was significant （P〈0.05）. The incidence of neonatal complications and death in middle group was obviously lower than that in the late group, the difference was significant （P〈0.05）.ConclusionsEarly diagnosis and timely treatment for gestational diabetes mellitus patients can decrease the rate of cesarean delivery, reduce maternal and infant complications in the perinatal period, and improve the neonatal prognosis.
