维持性血液透析患者血清骨保护素水平与血管钙化关系分析

The Relationship between OPG and Vascular Calcification in MHD

摘要 目的观察维持性血液透析患者血清骨保护素水平,并探讨其与血管钙化的相关性。方法选择2015年1月-2016年1月在潍坊市人民医院血液净化中心行规律血液透析治疗的60例患者作为研究对象,并选取30例健康体检者作为对照组,应用ELISA法测定各组患者血清骨保护素(OPG)水平;采用X线平片检查各组患者大动脉血管的钙化情况,并计算各个部位血管钙化积分。结果根据X线钙化评分将入选的60例MHD患者分为无钙化组(0分,21例,占MHD总人数的35%),轻度钙化组(-3分,共23例,占MHD总人数的38.3%),中度钙化组(-6分,共11例,占MHD总人数的18.3%),重度钙化组(-10分,共5人,占MHD总人数的8.4%),而对照组没有血管钙化的发生。在MHD患者中有血管钙化组OPG水平比无血管钙化组明显升高(P〈0.05);中、重度血管钙化组的OPG比无钙化或轻度钙化组明显升高(P〈0.05)。结论在MHD患者中血管钙化发生率高,血清OPG水平升高,OPG可能参与了血管钙化的发生及发展。 Objective To observe the maintenance hemodialysis patients' serum bone protection levels,and to explore its correlation with vascular calcification. Methods From January 2015 to January 2016,60 patients with hemodialysis in the blood purification center of weifang people's hospital were selected,and 30 cases of healthy physical examination were selected as control group,the application of ELISA method to determine each group patients serum levels of bone protection element( OPG); Groups were examined by using X-ray plain film of large arteries calcification,and calculated the parts vascular calcification score. Results According to the X-ray calcification score,60 patients with MHD were divided into non calcification group.( 0points,21 cases,accounting for 35% of the total number of MHD),mild calcification group( 1 - 3 points,a total of 23 cases,accounting for 38. 3% of the total number of MHD),moderate calcification group( 4 - 6points,a total of 11 cases,accounting for 18. 3% of the total number of MHD),severe calcification group( 7 - 10 points,a total of 5 people,accounting for 8. 4% of the total number of MHD),while the control group without the occurrence of vascular calcification. OPG levels in MHD patients were significantly higher than that in non vascular calcification group( P〈0. 05),and the OPG ratio was significantly higher in the group with severe vascular calcification than in the group with no calcification or slight calcification( P〈0. 05). Conclusion High incidence of vascular calcification in patients with MHD,elevated serum OPG levels,OPG may be involved in the occurrence of vascular calcifica-tion and development.
