脑梗死患者颈内动脉虹吸部钙化形态与血管狭窄的相关性研究 被引量:1

摘要 目的通过CT血管造影(CTA)观察脑梗死患者的颈内动脉虹吸部钙化形态与其该段血管狭窄的相关性。方法回顾性分析200例脑梗死患者的颈颅动脉CTA检查结果,所有病例根据CTA的图像将颈内动脉虹吸部钙化形态划分为0-4级,根据北美症状性颈动脉内膜切除术(NASCET)标准对颈内动脉虹吸部血管狭窄分级。比较颈内动脉虹吸部钙化形态与该段血管狭窄的关系。结果 200例脑梗死患者中,0级无钙化组、1级小点状钙化组、2级条状钙化组、3级半月状钙化组、4级环状钙化组发生血管狭窄率分别为5.6%,20.9%,66.7%,93.8%,95.8%.经卡方检验和Spearman等级相关分析显示,各组间颈内动脉虹吸部血管狭窄的发生率与钙化的形态比较,差异有统计学意义(P〈0.05),颈内动脉虹吸钙化形态与其该段血管的狭窄呈显著性相关(r=0.715,P〈0.05)。结论脑梗死患者的颈内动脉虹吸部钙化形态级别越高,越可以作为预测颈内动脉虹吸部血管狭窄的有效依据。 Objective To assess the relationship between carotid siphon calcification shape and vascular stenotic in patients with cerebral infarction on CTA images. Methods The CTA images of carotid siphon calcification in 200 cerebral infarction patients were analyzed retrospectively. All cases were divided into five grade by the shape of carotid siphon calcification. Furthermore,grade 0 - 1 were defined as low grade calcification,and grade 2 - 4 were defined as high grade calcification. The carotid siphon vascular stenotic was assessed based on North American Symptomatic Carotid Endarterectomy Trial. The relationship between internal carotid artery siphon of calcification shape and vascular stenotic was compared. Results Among 200 cases of patients with cerebral infarction: grade 0( no calcification) : 5. 6%,grade 1( small spotty foci of calcification) : 20. 9%,grade 2( a single calcification occupying 90°) : 66. 7%,grade 3( 90° - 270°) : 93. 8%,grade 4( 270° of arterial wall circumference) : 95. 8%. The obtained datas were analyzed by chi-square test and Spearman rank correlation analysis.Statistical analysis showed that carotid siphon vascular stenotic incidence was significantly associated with calcification shape( P〈0. 05). The vascular stenotic was significantly correlated with Carotid Siphon Calcification shape( r = 0. 715,P〈0. 05). Conclusion The higher level of calcification in the siphon of internal carotid artery in patients with cerebral infarction,the more effective it can be used to predict the vascular stenosis of internal carotid artery siphon.
