后腹腔镜肾囊肿去顶术并脂肪填塞囊腔术在临床中的应用

Clinical Application of Retroperitoneal Laparoscopic Capsulotomy for Renal Cyst and Liposuction

目的探讨后腹腔镜肾囊肿去顶术并带蒂脂肪填塞术在术后预防肾囊肿复发的疗效。方法将80例单纯性肾囊肿患者随机分为两组：治疗组40例患者采用肾囊肿去顶术并脂肪填塞囊腔术治疗,对照组仅采用传统的后腹腔镜肾囊肿去顶术,观察对比两组手术时间、术中出血量和术后并发症,随访术后复发的情况。结果治疗组术后无并发症出现,对照组有1例出现肾脏积液并感染;术后随访3-24个月,治疗组出现残腔存留或复发患者1例（2.5%）,对照组4例（10.0%）,治疗组明显低于对照组。结论后腹腔镜肾囊肿去顶术并带蒂脂肪填塞术可有效降低术后并发症发生率及囊肿复发率。

Objective To study the effect of retroperitoneal laparoscopic cystectomy and renal embolization in prevention of renal cyst recurrence. Methods Eighty patients with simple renal cysts were randomly divided into two groups： 40 patients were treated with renal cysts and liposuction,and the control group was treated with conventional retroperitoneoscopic renal cyst. The operation time,blood loss and postoperative complications were observed and compared. The recurrence of the patients was followed up. Results There were no complications in the treatment group and 4 cases in the control group. In the follow-up period of 3 - 24 months,one patient（ 2. 5%） survived or relapsed in the treatment group,and the control group was 4 cases（ 10. 0%）,the treatment group was significantly lower than that of control group. Conclusion Retroperitoneal laparoscopic cystectomy and pedicle fat packing can effectively reduce postoperative complications and cyst recurrence rate.
