丙种球蛋白联合持续血液净化对小儿重症脓毒症患者血清降钙素原及危重评分的影响

Effect of Gamma Globulin Combined with Continuous Blood Purification on the Serum Calcitonin Level and Critical Score of Children with Severe Sepsis

目的:探究丙种球蛋白联合持续血液净化对小儿重症脓毒症患儿血清降钙素原及危重评分的影响。方法:选择我院收治的重症脓毒症小儿患者37例,随机划分成实验组和对照组。对照组18例给予持续血液净化治疗;实验组19例予持续血液净化及注射丙种球蛋白治疗。比较两组临床疗效及治疗前后血清降钙素原及危重评分水平的变化。结果:实验组治疗后总有效率(94.7%)显著高于对照组(61.1%),差异存在统计学意义(P<0.05)。治疗后,两组患者的血清降钙素原水平均较治疗前显著降低,小儿危重症评分(PCIS)水平均明显升高;与对照组比较,实验组血清降钙素原水平较低,PCIS评分较高,差异有统计学意义(P<0.05)。结论:丙种球蛋白联合持续血液净化可有效升高小儿重症脓毒症患者的PCIS评分,临床疗效好,其作用可能与降低血清降钙素原水平相关。

Objective： To investigate the effect of gamma globulin combined with continuous blood purification on the serum cal- citonin level and critical score of children with severe sepsis. Methods： 37 children with severe sepsis in our hospital were selected and randomly divided into the control group and the experiment group. 18 cases in the control group were treated by continuous blood purification, l 9 cases in the experiment group were treated by continuous blood purification and injection of immunoglobulin. The serum level of calcitonin and critical score were compared between two groups before and after treatment. Results： The total effective rate of experiment group （94.7%） was higher than that of the control group （61.1%） （P〈0.05）. The serum calcitonin levels in two groups were decreased after treatment, the PCIS levels were creased after treatment. Compared with the control group, the serum calcitonin level was lower, the PCIS level was higher （P〈0.05）. Conclusion： Gamma globulin combined with continuous blood purification could effectively increase the PCIs scores of children with severe sepsis, which had good clinical effect and might be related to the decreased serum calcitonin level.
