腔内超声多模式检查评估宫颈癌放化疗效果初步报告

Evaluation of the Effects of Radiotherapy and Chemotherapy in Cervical Cancer by Multi Mode Endo-luminal Ultrasound

目的:探讨腔内超声多模式检查在宫颈癌放化疗效果评估中的应用价值。方法:对17例晚期宫颈癌患者进行了治疗前与治疗后的多模式超声检查,比较治疗前后的影像差异;结果:17例病例放化疗后宫颈体积缩小,形态恢复正常,治疗前后宫颈大小有显著差异(P<0.05),达到完全缓解的评价标准;治疗后宫颈硬度减低,宫颈弹性评分下降:治疗前15例宫颈弹性评分为5分,2例弹性评分为4分,治疗后17例宫颈弹性评分均减低,评分为3分;17例患者治疗前宫颈超声造影增强时间早于宫体,增强强度高于宫体,消退速度快于宫体,治疗后超声造影表现为与宫体肌层同步强化,无异常灌注区,治疗前后宫颈超声造影表现差异明显。结论:多模式超声检查的联合应用可以为宫颈癌放化疗疗效的临床评估提供更多有价值的信息,超声造影更具价值。

Objective： To evaluate the clinical value of multi-patterns Transvaginal ultrasonography in the effects of radiotherapy and chemotherapy of cervical cancer. Methods： 17 cases of patients with advanced cervical cancer were enrolled, the images of multi-patterns ultrasonography were compared before and after treatment. Results： The cervical sizes were statistically different before and after treatment （P 〈0.05）. After the treatment, the cervix had a lower hardness and the elasticity score was reduced. Before the treatment, the value of strain ratio was 5 in 15 cases, and the value of strain ratio was 4 in 2 cases. After the treatment, the value of strain ratio was 3 in all of the cases. In contrast-enhanced ultrasound, enhanced time of the cervix was earlier than the uterine myometrium in 17 cases, and the intensity of the cervix was more strengthened. The fading in cervix was earlier than the uterine myometrium in 17 cases. After the treatment, the cervix and uterine myometrium were synchronized with the contrast-enhanced ultrasound in 17 cases, no abnormal perfusion areas were observed. The contrast-enhanced ultrasound in cervix was statistically different before and after treatment. Conclusions： Combined application of multi-patterns Transvaginal ultrasonography could provide more valuable information for cervical cancer in effects of radiotherapy and chemotherapy.
