外剥内套齿线保留术治疗Ⅱ、Ⅲ度混合痔的临床效果

Clinical efficacy of external stripping plus internal loop ligature with preserved dentate line operation in treating Ⅱ and Ⅲ degree mixed hemorrhoid

目的探讨外剥内套齿线保留术治疗Ⅱ、Ⅲ度混合痔的临床疗效。方法 144例Ⅱ、Ⅲ度混合痔患者随机均分为治疗组和对照组。治疗组采用外剥内套齿线保留术;对照组采用外剥内扎术。比较两组治疗总有效率、术后VAS疼痛评分、创面愈合时间和住院时间。结果两组治疗总有效率均为100%。与对照组比较,治疗组术后第3和7天VAS疼痛评分低[(3.6±2.8)分vs.(4.3±2.0)分和(2.8±1.6)分vs.(3.6±1.6)分],创面愈合时间早[(23.3±2.0)d vs.(29.2±3.7)d],住院时间短[(4.8±1.3)d vs.(9.3±3.7)d](P均<0.05)。结论与传统的外剥内扎术相比,外剥内套齿线保留术治疗Ⅱ、Ⅲ度混合痔具有组织损伤小、术后并发症少和恢复快的优势。

Objective To study the efficacy of external stripping plus internal loop ligature with preserved dentate line operation in the treatment of Ⅱ and Ⅲ degree mixed hemorrhoid. Methods A total of 144 patients with Ⅱ and Ⅲ degree mixed hemorrhoid was equally randomized into two groups of A（treated with external stripping plus internal loop ligature with preserved dentate line operation） and B（treated with Milligan-Morgan operation）. The effectiveness rate, postoperative VAS pain score, wound healing time and hospital stay were compared between two groups. Results The overall effectiveness rate was 100% in both groups. Compared with group B,VAS pain score on the 3rd and 7th day after operation was lower[（3. 6±2.8） points vs. （4.3±2.0） points and （2.8±1.6） points vs. （3.6± 1.6） points] （P〈0. 05）, the wound healing time[（23.3 ± 2.0） d vs. （29.2 ± 3.7） d] and hospital stay[（4.8 ± 1.3） d vs. （9.3 ± 3.7） d] were shorter in group A（P〈0. 05）. Conclusion Compared with Milligan-Morgan operation, external stripping plus internal loop ligature with preserved dentate line surgery has the advantages of smaller tissue damage, fewer complications and faster postoperative recovery in the treatment of Ⅱ and Ⅲ degree mixed hemorrhoid.
