摘要 目的:寻找胃癌化疗药物、用药人群与药品不良反应间潜在的关联性,为临床合理用药提供参考。方法:收集109家药品生产企业2010~2014年生产的8种胃癌化疗药物不良反应报告数据,运用数据挖掘Apriori算法构建关联模型,使用Clementine 14.1进行统计分析。结果:共收集有效报告10 572份,男女比例为2.11∶1。8种抗肿瘤药物的不良反应主要为恶心、呕吐、腹泻、局部麻木和骨髓抑制,主要发生于40~65岁患者。男性与恶心、呕吐、骨髓抑制、局部麻木、腹泻和白细胞减少的关联性较强,女性与胸闷、恶心和呕吐的关联性较强。奥沙利铂与恶心、呕吐、局部麻木、骨髓抑制、白细胞减少、腹泻的关联性较强。多西他赛与胸闷、腹泻的关联性较强。氟尿嘧啶出现骨髓抑制、血小板减少的可能性较大。卡培他滨与乏力、腹泻的关联性较强。顺铂与呕吐、血小板减少,紫杉醇与肝功能异常、呕吐、乏力的关联性较大。结论:临床工作者在患者用药过程中应关注联合用药、用药人群与药品不良反应间的关联性。 Objective: To find the potential correlation between gastric cancer chemotherapy drugs,patient group and the adverse drug reactions. To evaluate the safety of the drug and guide clinical use. Methods: Collected gastric cancer adverse drug reaction reports of 109 pharmaceutical producing enterprise from 2010 to 2014,using data mining technology to analyze,build correlation model using apriori algorithm,apply clementine 14. 1 for statistical analysis. Results: 10 572 effective reports and statistical results of adverse drug reactions reports were collected,the ratio of male and female was2. 11∶ 1. The main performance of adverse drug reactions were nausea,vomiting,diarrhea,monoanesthesia and myelosuppression,mainly accured between 40 to 65 years old patients. Nausea,vomiting,monoanesthesia,myelosuppression,diarrhea and leukopenia showed strong relevance with male; chest stufly,nausea and vomiting had strong associated with female. Nausea,vomiting,monoanesthesia,myelosuppression,leukopenia and Diarrhea were more likely to occurred when taking oxaliplatin. Chest stufly and diarrhea were mainly caused by docetaxel. Myelosuppression and thrombocytopenia is more likely to occurred when taking fluorouracil. Debilitationand diarrhea were more likely to occurred when taking capecitabine; Vomiting,thrombocytopenia were mainly caused by cisplatin. Liver dysfunction,vomiting,Debilitation were more likely to occurred when taking Paclitaxel. Conclusion: Physicians should pay attention to correlation between combination medication,population of exposure and adverse drug reaction when processing medicine.
