护士大五人格与机械通气患者床头抬高依从性的关系研究

A study on the relationship between ICU nurses personality characteristics and the compliance of head-of-bed elevation in patients with mechanical ventilation

目的探讨ICU护士人格与抬高机械通气患者床头依从性的关系。方法采用大五人格量表分析ICU护士的人格特征;运用自制护士抬高机械通气患者床头依从性调查表,评估护士抬高机械通气患者床头依从性。数据应用SPSS 17.0进行统计分析。结果人口学资料(年龄、性别、职称等)对床头抬高依从性的影响无统计学差异(P>0.05)。大五人格中神经质、开放性、严谨性与床头抬高依从性存在负相关、正相关、正相关关系(r=-0.711,P<0.01;r=0.527,P<0.01;r=0.714,P<0.01)。神经质、开放性、严谨性在logistic回归方程中对床头抬高依从性的影响具有统计学意义(P<0.05),并且严谨性越高,床头抬高依从性越高;神经质和开放性越高,则床头抬高依从性越低。神经质、开放性、严谨性能解释床头抬高依从性72.2%的变异。结论人格中神经质、开放性、严谨性为影响护士对机械通气患者床头抬高依从性的关键因素。

Objective To explore the relationship between ICU nurses personality characteristics and the compliance of head-of-bed eleva- tion in patients with mechanical ventilation.Methods ICU nurses＇ personality traits were adopted by big five personality inventory.Using self- designed evaluation sheet to assess the nurses＇ compliance to head-of-bed elevation as one effective action of the ventilator-associated pneu- monia（VAP） prevention and control measures in patients with mechanical ventilation.All statistical analyses were performed using SPSSvl7.0 software （SPSS, Inc. Chicago , IL, USA）.Results There was no statistical significance （P〉0.05）in the influence of demographic data on the compliance of head-of-bed elevation.There was a significant negative correlation （ r = - 0.711, P 〈 0.01 ） between neurotieism and＇ the compli- ance of head-of-bed elevation in the big five personality, a significant positive relationship between openness and the compliance of head-of- bed elevation （r= 0.527 ,P〈0.01 ） , and a significant positive correlation between conscientiousness and the compliance of head-of-bed eleva- tion（ r= 0.714,P〈0.01 ）.Neuroticism,openness and conscientiousness affected the compliance of head-of-bed elevation in the logistic regres- sion equation with statistical significance （P〈0.05）.The higher the conscientiousness, the better the compliance of head-of-bed elevation.The higher the neuroticism and openness, the lower the compliance of head-of-bed elevation.Neureticism, openness, and conscientiousness per- formance could explain 72.2% variation on the compliance of head-of-bed elevation.Conclusion Neuroticism, openness and conscientious- ness were the three key factors in the big five personality traits of ICU nurses influenced the compliance to head-of-bed elevation in patients with mechanical ventilation.
