高同型半胱氨酸血症与年轻原发性高血压患者肾功能的关系

The relationship between hyperhomocysteinemia and renal function in young hypertensive patients

目的分析高同型半胱氨酸血症与年轻原发性高血压患者肾功能的关系。方法选择原发性高血压患者93例,根据血浆同型半胱氨酸水平分为H型高血压组(血浆同型半胱氨酸≥10μmol/L)45例与普通高血压组(血浆同型半胱氨酸<10μmol/L)48例,比较2组患者血肌酐(Cr)、血尿素氮(BUN)、肾小球滤过率(e GFR)、24 h尿蛋白及24 h尿微量白蛋白。并分析血浆同型半胱氨酸水平与24 h尿微量白蛋白的相关性。结果 2组患者Cr、BUN、e GFR、24 h尿蛋白比较差异均无统计学意义(P>0.05);H型高血压组24 h尿微量白蛋白明显高于普通高血压组(P<0.05)。血浆同型半胱氨酸水平与24 h尿微量白蛋白存在正相关(r=0.352,P<0.05)。结论高同型半胱氨酸血症与年轻原发性高血压患者极早期肾功能损害呈正相关。

Objective To analyze the relationship between hyperhomocysteinemia and renal function in young hypertensive patients. Methods A total of 93 patients with primary hypertension were collected in the study,these patients fell into two parts by the level of serum homocysteine,one was H-type hypertension group（ plasma homocysteine levels was greater than or equal to 10 μmol / L） with 45 cases,and another was common hypertension group（ plasma homocysteine levels was less than10 μmol / L） with 48 cases. The serum creatinine（ Cr）,blood urea nitrogen（ BUN）,glomerular filtration rate（ e GFR）,24 hours＇ urinary protein and urinary microalbumin of 24 hours were compared between the two groups,and correlation between plasma homocysteine level and 24 hours＇ urinary microalbumin was analysed. Results The differences of Cr,BUN,e GFR,24 hours＇ urinary protein between the two groups were not statistically significant（ P〉 0. 05）. 24 hours＇ urinary microalbumin of H-type hypertension group was significantly higher than that of normal hypertension group（ P 〈0. 05）. There was a positive correlation between plasma homocysteine level and 24 hours＇ urinary microalbumin（ r = 0. 352,P 〈0. 05）. Conclusion Hyperhomocysteinemia is positively correlated with early renal dysfunction in young patients with essential hypertension.
