摘要
目的探讨超声造影和实时组织弹性成像在乳腺肿块鉴别诊断的应用价值。方法选取2014年2月-2016年2月在新疆医科大学附属肿瘤医院就诊的126例患者共180个乳腺肿块,随机分为试验组1、试验组2和试验组3,每组60个乳腺肿块,试验组1采用实时组织弹性成像检查,试验组2采用超声造影检查,试验组3采用实时组织弹性成像联合超声造影检查,以病理学检查结果为金标准,观察3组检查方法对乳腺肿块的诊断价值。结果超声造影下的良性肿块多呈等增强,点线状和结节状增强,增强边界多平滑;恶性肿块以不均匀的高增强为主,增强边界不清或欠清。实时组织弹性成像联合超声造影检查诊断敏感性、特异性、准确性明显高于单纯超声造影检查及实时弹性成像检查,3组相比差异具有统计学意义(P<0.05)。结论对乳腺肿块采用超声造影联合实时组织弹性成像检查的结果比单一的检查方法更准确。
Objective To study the application of contrast-enhanced ultrasonography and the real-time tissue elastography in differential diagnosis in breast lumps. Methods 180 cases with hyperplasia of mam- mary glands in Xinjiang tumor hospital from February 2014 to February 2016, were retrospectively analyzed and were randomly divided into experimental group 1, experimental group 2 and experimental group 3 (n = 60). Experimental group 1 used real-time tissue elasticity imaging, Experimental group 2 used contrast-enhanced ultrasonography. The experimental group 3 used real-time tissue elastography in combination with contrast-enhanced ultrasonography examination. The diagnostic value of the 3 methods in the diagnosis of breast lumps was evaluated by the pathological examination results. Results Under the contrast-enhanced ultrasonography of benign lumps are not enhance, linear and nodular enhancement, and the strengthen border are more smooth. Malignanttumor is given priority to with uneven high enhance- ment, and the enhance boundary is not clear or owed. In Real-time tissue elastography combination with contrast-enhanced ultrasound inspection group, diagnostic sensitivity, specificity and accuracy were significantly higher than that of ultrasonic imaging or real-time elastic group, and there was statistically signifi- cant difference (P〈0.05). Conclusion The results of contrast-enhanced ultrasound combined with realtime tissue elastography in breast lumps are more accurate than the single method.
