新疆汉、维吾尔、哈萨克族原发性高血压患者血清TGF-β1水平与血压的相关性分析 被引量:5

摘要 目的探讨新疆地区汉族、维吾尔族、哈萨克族原发性高血压(EH)患者血清转化生长因子-β1(TGF-β1)水平的差异性及其与血压的相关性。方法纳入EH患者288例,其中汉族131例,维吾尔族65例,哈萨克族92例,均进行一般体格检查、诊室血压及24h动态血压监测(24hABPM),抽取空腹静脉血,测定其空腹血糖、血脂、肾功等指标,采用酶联免疫吸附测定(ELISA)法检测所有入选者外周血中TGF-β1水平,并根据患者入院时所测诊室血压水平进行分级,依据《中国高血压防治指南2010》分为4个组,分别为诊室血压正常EH组、诊室血压1级EH组、诊室血压2级EH组、诊室血压3级EH组,比较3个民族EH患者间性别比例、年龄、体质指数、血尿素氮、血肌酐、空腹血糖、动态血压各参数和按诊室血压分级后3个民族EH患者血清TGF-β1水平。结果哈萨克族EH患者白昼平均舒张压(dDBP)与夜间平均舒张压(nDBP)水平均高于汉族和维吾尔族(P<0.05)。维吾尔族EH患者血清TGF-β1水平最高,哈萨克族EH患者最低。汉族与哈萨克族EH患者的血清TGF-β1水平随血压分级的升高呈逐渐增加趋势;简单相关分析及控制年龄、性别、体质指数后的偏相关分析均显示汉族患者血清TGF-β1水平与舒张压呈正相关(P<0.05)。简单相关分析显示维吾尔族患者血清TGF-β1与nDBP呈负相关,控制年龄、性别、体质指数后的偏相关分析显示血清TGF-β1水平与24h平均收缩压(24hSBP)、(dDBP)、白昼平均收缩压(dSBP)、24h平均舒张压(24hDBP)、夜间平均收缩压(nSBP)、(nDBP)均呈负相关(P<0.05)。哈萨克族中TGF-β1与血压之间未见相关性。结论汉族、维吾尔族、哈萨克族EH患者血清TGF-β1水平存在民族差异;血清TGF-β1与高血压分级水平相关。 Objective To investigate the differences of the serum Transforming Growth Factor betal (TGF-β1) levels in different ethnic groups (Han, Uyghur and Kazak in Xinjiang) with essential hypertension (EH) and their correlation with blood pressure. Methods A total of 288 EH patients (131 Han, 65 Uy-ghur, 92 Kazak) were enrolled in this study. All were given physical, clinic blood pressure examinations as well as 24h ambulatory blood pressure monitoring (24hABPM). The peripheral fasting venous blood of all subjects were obtained to detect their general biological indexes, including fasting blood glucose (FBG), lipid, urea nitrogen, creatinine as well as serum TGF-β1 concentration by ELISA. Based on the clinic blood pressure, patients were divided into four groups according to Guidelines for prevention and treatment of hypertension in China 2010: EH group of normal clinic blood pressure, EH group of the first grade clinic blood pressure, EH group of the second grade clinic blood pressure, EH group of the third grade clinic blood pressure. To compare the gender percentage, age, BMI, urea nitrogen, creatinine, FBG, parameters of 24hABPM in 3 ethnic groups with EH, and the serum TGF-β1 concentration in the 4 groups. Results The dDBP and nDBP levels in Kazak EH were higher than those in Han and Uyghur subjects. The concentration of serum TGF-β1 was highest in Uyghur, while lowest in Kazak. The serum TGF-β1 level was gradually increasing with the elevation of blood pressure classification in Han and Kazak. In Han, the serum TGF-β1 level was correlated with DBP (P〈0.05) by both Pearson and Partial Correlation anal- ysis adjusted by age, gender and body mass index (BMI). In Uyghur, the serum TGF-β1 was just correla ted with nDBP (P (0.05) by Pearson Correlation, while controlled age, gender and BMI, partial correlation demonstrated that TGF-β1 was correlated with 24hSBP, 24hDBP, dSBP, dDBP, nSBP, nDBP (P〈0.05). There was no significant correlation between TGF-β1 and blood pressure in Kazak. Conclusion There are differences of serum TGF-β1 levels in different ethnic groups (Han, Uyghur and Kazak) with EH. The serum TGF-β1 is associated with the level of hypertension.
