伽玛刀治疗垂体腺瘤159例疗效分析 被引量:4

Application of gamma knife radiosurgery for the treatment of pituitary adenoma: report of 159 cases

摘要 目的 探讨伽玛刀(γ-刀)治疗垂体腺瘤的适应证和疗效。方法 随访垂体腺瘤经γ-刀治疗 6个月以上的病例159例(男58例,女101例),其中无功能性腺瘤45例,内分泌型腺瘤114例。无功能性腺瘤治疗周边剂量10~18Gy,中心剂量25~36Gy;内分泌型腺瘤周边剂量12~30Gy,中心剂量35~65Gy。结果 135例治疗后症状减轻,影像学检查显示有肿瘤坏死、病灶缩小、囊性变等表现;17例无明显影像学改变;7例分别因肿瘤卒中、囊性变或病灶增大等原因而接受手术治疗。肿瘤发展得到控制152例(95%),无并发症发生。结论γ-刀是治疗垂体腺瘤的有效手段,安全性高,并可成为治疗垂体微腺瘤的首选方法。 Objective To determine the indications and evaluate the effects of gamma knife surgery (GKS) for pituitary adenoma. Methods Clinical data of 159 (58 male, 101 female) pituitary adenoma cases with over 6-month period after GKS were reviewed retrospectively and the follow-up results were analyzed, among them 45 cases suffered from nonfunctional pituitary adenoma, while 114 cases hormone-producing adenoma. The margin doses of 10 - 18 Gy and maximum doses of 25 -36 Gy were given to the nonfunctional adenoma, while the doses of 12 - 30 Gy to the border and 35 - 65 Gy to the centre of the tumor were given to the hormone-producing adenoma. Results Clinical symptoms were improved in 135 cases, whose imaging finding showed central necrosis of tumor or cystic formation and reduction of tumor mass. Seventeen cases had no evident changes in the images. Seven cases underwent surgery 6 months after GKS due to the invasive growth of adenoma , capsule-like formation or developing apoplexy in the tumor. The tumor control was achieved in 152 cases (95.6% ). All cases had no complications associated with GKS. Conclusion GKS is an effective and safe way to treat the patients with pituitary adenoma, even can become the first choice for those with pituitary microadenoma.
