两种结膜瓣缝合位置对翼状胬肉疗效的影响

Clinical comparison of two kinds of conjunctival flap Stitching Position in treatment of pterygium

目的比较两种结膜瓣缝合位置对翼状胬肉疗效的影响。方法选取2014年1月-2016年4月在佛山市顺德区第二人民医院行翼状胬肉手术患者45例(45眼),随机分为观察组和对照组观察组23例(23眼)行翼状胬肉切除术角膜缘上皮侧缝于角膜缘处对照组22例(22眼)行翼状胬肉切除术角膜缘上皮侧缝于角膜缘外2 mm处。观察两组的结膜瓣成活率、术后复发率、角膜上皮修复时间。结果两组患者结膜植片均成活成活率为100.00%。角膜上皮愈合时间,观察组(3.22±0.85)d,对照组(4.36±1.05)d,两组差异有统计学意义(P<0.01)。观察组复发1例(1眼),复发率为4.34%,对照组复发1例(1眼),复发率为4.55%,两组差异无统计学意义(P>0.05)。结论翼状胬肉切除术联合带角膜缘上皮结膜瓣移植,将角膜缘上皮侧直接缝于角膜缘处能更快的修复角膜上皮,重建眼表组织。

Objective Compare the effect of two kinds of conjunctival flap position in treatment of pterygium.Methods 45 cases（45 eyes） of pterygium surgery in Shunde District Second People＇s Hospital from January 2014 to April 2016 were selected and randomly divided into observation group and control group.The observation group of 23 cases（23 eyes）,performed the pterygium excision,were sutured it on the corneal margin;The control group of 22 cases（22 eyes）,performed the pterygium excision,were sutured it on the position which 2 mm out of the corneal margin.The survival rate of conjunctival flap,the recurrence rate of surgery and the repairing time of cornea epithelium in two groups were observed.Results The two groups of patients with conjunctival graft all survived,the survival rate was 100.00%.The repairing time of corneae epithelium ＇.the observation group was（3.22±0.85） days,the control group was（4.36±1.05） days,there was significant difference（P〈0.01）.Reoccurrence rate：the observation group was 4.34%（1 case） with 4.55%（1 case） in the control group,there was no significant difference between the two groups（P〈0.05）.Conclusion Corneae epithelium can repair faster and quicken the ocular surface repairing when suture the limbal epithelial autograft right on the corneal margin.
