综合康复治疗在创伤后肘关节功能障碍中的应用

Comprehcnsive rehabilitation in the management of traumatic elbow fracture with elbow dysfunction

目的探讨综合康复治疗在创伤．J生肘部骨折后肘关节功能障碍中的疗效。方法自2014年6月至2015年6月我们共收治68例肘关节骨折后功能障碍患者。随机进入实验组和对照组，每组34例。实验组患者采用综合康复治疗，即中药熏洗＋微调式静态牵伸支具＋关节松动术＋主动训练；对照组仅采用关节松动术治疗。所有患者分别于治疗前和治疗后2、6、12周采用视觉模拟评分法（visual analogue scale，VAS）进行疼痛评分及肘关节功能Mayo评分。结果治疗后2、6、12周两组患者VAS疼痛评分和肘关节功能Mayo评分较治疗前均有所改善，差异均有统计学意义（P〈0．05）；同时间点组间对比，实验组患者评分均显著高于对照组，差异均有统计学意义（P〈0．05）。结论在创伤性肘关节骨折后关节功能障碍中，采用综合康复治疗不仅能够改善肘关节功能，还可以大大提升康复效果，值得临床推广应用。

Objective To explore the application of comprehensive rehabilitation therapy in elbow joint dysfunction resulted from traumatic elbow fracture. Methods Sixty-eight patients with elbow dysfunction after fractures were treated in our hospital between June 2014 and June 2015. The patients were randomly divided into the comprehensive treatment group and control group with 34 patients each. Patients in the comprehensive treatment group underwent the comprehensive rehabilitation therapy （traditional Chinese herbal fumigation＋fine-tuning static drawing limp＋arthrosis mobilization＋active training）. Patients in the control group were treated scale （VAS） pain score and Mayo elbow performance with arthrosis mobilization only. Visual analogue score were assessed before and 2, 6 and 12 weeks after the treatment, respectively. Results After 2, 6 and 12 weeks of treatment, VAS pain score and Mayo elbow performance score were improved in patients of both groups compared with pre-treatment. The differences were statistically significant （P〈0.05）. The scores of the comprehensive treatment group were all significantly higher than scores of the control group （P〈0.05）. Conclusion Comprehensive rehabilitation therapy can not only improve the function of the elbow, but also greatly improve the efficacy of rehabilitation, which is worth to be popularized and applied clinically.
