女性生理周期心电图T波改变的特点及临床意义 被引量:11

Characteristics and clinical significance of changes of electrocardiogram T-wave in female physiologic cycle

摘要 目的研究女性生理周期中心电图T波改变的特点,并探讨其与雌激素水平的相关性。方法选取2014年3月至2015年5月间于我院进行常规体检中出现心电图T波改变的健康育龄女性100例,分别行常规12导联心电图检查,测定血清中雌二醇水平,并进行相关性分析。结果育龄女性生理周期(月经期、子宫内膜增生期和子宫内膜分泌期)心电图T波改变的主要特征为低平或倒置,而排卵期心电图T波正常(变成直立);排卵期雌二醇水平为(314.6±95.8)pmol/L,明显低于月经期的(336.4±117.5)pmol/L、子宫内膜增生期的(438.4±135.4)pmol/L以及子宫内膜分泌期的(342.1±119.6)pmol/L,差异均具有统计学意义(P<0.05);Spearman相关性分析表明,育龄女性生理周期雌激素水平变化与心电图T波改变具有显著正相关性(r=0.714,P<0.05)。结论健康育龄女性生理周期内心电图存在一定的T波改变,其可能与生理周期中雌激素水平变化相关。 Objective To study the change characteristics of electrocardiogram(ECG) in the female physiological cycles, and then to discusses its correlation with estrogen level. Methods A total of 100 cases of healthy women of childbearing age, who were found T-wave changes during the routine physical examination in our hospital from March2014 to May 2015, were selected as the research subjects. Routine 12-lead electrocardiogram inspection was performed,the levels of serum estradiol(E2) levels was determined. The correlation analysis was carried out. Results The main characteristics of T wave changes of electrocardiogram in the female physiological cycles(menstrual period, endometrial hyperplasia and endometrial secretory phase) was low or inverted, while the ECG T-wave of ovulation phase was normal(into the upright). The level of estradiol during ovulation was(314.6±95.8) pmol/L, which was significantly lower than(336.4±117.5) pmol/L,(438.4±135.4) pmol/L,(342.1±342.1) pmol/L of menstrual period, endometrial hyperplasia and endometrial secretory phase, respectively(P〈0.05). Spearman correlation analysis showed that there was a significant positive correlation between the change of estrogen level and ECG T-wave in women of childbearing age(r=0.714, P〈0.05). Conclusion There were some physiological cycle T wave change in the healthy women during the physiological cycle, which may be related to the change of the estrogen level in the physiological cycle.
