神经内镜治疗脑出血的疗效及经济学价值

Clinical effect and economic value of the nerve endoscope in the treatment of intracerebral hemorrhage

目的探讨脑出血(intracerebral hemorrhage,ICH)患者采用神经内镜治疗的临床效果,及其临床经济学价值。方法选取本院2014年1月~2015年12月收治的106例ICH患者进行回顾性分析,根据其手术方式不同分为显微镜组58例与内镜组48例,显微镜组给予小骨窗显微镜下血肿清除术,内镜组给予神经内镜下血肿清除术。结果内镜组患者的手术时间、术中出血量、切口长度、血肿清除率、血肿消失时间以及并发症发生率均好于显微镜组(P<0.05);内镜组患者的GOS预后评分显著优于显微镜组(P<0.05);内镜组患者的住院时间与治疗费用均低于显微镜组(P<0.05)。结论采用神经内镜治疗ICH患者能够减少手术损伤,促进预后恢复,并且具有良好临床经济价值。

Objective To discuss the effect and economic value of the nerve endoscope in the treatment of intracerebral hemorrhage（ ICH）. Methods 106 patients of ICH in our hospital from January 2014 to December 2015 were analyzed retrospectively. According to the operation method,58 patients were microscope group and 48 patients were endoscopic group. Who were treated with small bone window operation under microscope and hematoma aspiration under nerve endoscope. Results The operation time,the amount of bleeding,the length of incision,the rate of hematoma evacuation,the disappearance time and complication rate of patients in endoscopic group were better than those in microscope group（ P〈0. 05）. The GOS scores of patients in endoscopic group was were significantly better than that in microscope group（ P〈0. 05）. The length of hospital stay and treatment cost of patients in the endoscopic group were lower than that in the microscope group（ P〈0. 05）. Conclusion Endoscopic treatment for ICH patients can reduce surgical injury,promote the restoration of prognosis,has good clinical economic value.
