负压封闭引流技术在皮肤原位回植中的应用 被引量:4

摘要 目的探讨撕脱无血供皮肤利用反取皮打薄原位回植联合VSD(负压封闭引流技术)治疗的疗效。方法选择我院创伤骨科自2012年1月至2016年1月收治的59例四肢皮肤撕脱无血供的患者,急诊行反取皮打薄原位回植术,在患者及家属同意的情况下随机选择实验组:原位回植联合VSD技术30例,对照组:原位回植加压包扎或打包固定患者29例,同一治疗组医生手术,术后随访观察患者的皮肤成活情况、伤口感染及外观情况。结果所有患者均取得随访,随访时间2-6个月,平均3个月,实验组全部植皮成活27例,3例植皮小部分坏死,伤口感染1例,通过换药后瘢痕愈合,无病例植皮全部坏死,所有病例瘢痕增生少,外观好;对照组植皮全部成活22例,植皮部分坏死5例,植皮全部坏死2例,伤口感染4例,其中有5例再次植皮,并植皮成活,1例瘢痕愈合,1例皮瓣修复后愈合,有4例见明显增生的瘢痕组织,外观差。结论急诊反取皮原位回植联合VSD技术治疗皮肤撕脱伤较反取皮原位回植加压包扎成活率高,感染率低,外观更佳。
