冠心病病人监护病房停留时间的影响因素分析

Influencing factors of length of stay of patients with coronary heart disease in CCU

[目的]探讨影响冠心病病人在CCU停留时间延长的因素。[方法]采用横断面研究收集2014年1月—6月于华西医院心内科CCU接受治疗并且符合纳入标准的冠心病病人资料并进行分析。[结果]影响病人CCU停留时间的因素包括病人年龄、入住CCU方式、基础疾病权重评分、是否合并肺部感染、慢性肾功能不全、心律失常等疾病以及是否接受无创/有创呼吸机、中心静脉置管、胃管、尿管、主动脉内球囊反搏(IABP)治疗、血液透析、抢救等治疗。[结论]冠心病病人在CCU停留期间,医务人员在治疗冠心病的同时,应积极控制肺部感染,纠正心律失常,治疗稳定后及早结束无创/有创呼吸机、拔除中心静脉置管、胃管、尿管等,以缩短CCU停留时间。

Objective： To probe into the factors influencing the prolonging length of stay in Coronary heart dis-ease Care Unit（CCU） of patients with coronary heart disease. Methods： Data of coronary heart disease patients who received CCU in the department of Cardiology in Huaxi Hospital from January to June in 2014 were col-lected and analyzed by cross - sectional study. Results： The factors influencing the length of stay in CCU included patients age, check in style to CCU, weighted score of basic disease, whether combined with pulmonary infec-tion, chronic renal insufficiency,arrhythmia and whether to accept noninvasive or invasive ventilator, central vein catheter,gastric tube,urinary catheter, intra - aortic balloon counterpulsation（IABP） treatment, hemodialysis, rescue and other treatment. Conclusions： In order to shorten the length of stay in CCU,the medical staff should actively control the pulmonary infection, correct the arrhythmia, stop using the noninvasive/invasive ventilator as soon as possible after treatment, and remove the central venous catheter,gastric tube,and urinary catheter etc.
