腰椎间盘突出症住院患者术前自我效能与生活质量的相关性分析

Correlation study of self-efficacy and quality of life in preoperative hospitalized patients with LDH

目的了解腰间盘突出症住院患者术前自我效能和生活质量现状，并分析两者间的相关性。方法2015年12月—2016年2月选取上海市4家三级甲等医院收治的138例腰椎间盘突出症住院患者，采用一般资料调查表、生活质量量表（SF36）及一般自我效能感量表调查患者术前自我效能和生活质量现状，并分析两者之间的相关性。结果腰椎间盘突出症住院患者术前自我效能得分为（24.15±6.57）分，低于国内常模，差异有统计学意义（t=-7.671，P〈0.01）；生活质量得分为（53.71±19.38）分，不同年龄、职业性质、工作姿势、腰椎负荷及教育程度患者生活质量差异有统计学意义（F值分别为4.119，6.207，5.972，5.064，4.703；P〈0.01）。患者的一般自我效能与生活质量中的生理机能、生理职能、躯体疼痛、健康总况、社会功能及情感职能6个维度呈正相关（r=0.186~0.389；P〈0.05）。结论腰椎间盘突出症住院患者的自我效能、生活质量均低于常模，护理人员应注意采取有效的护理措施提高腰椎间盘突出症住院患者的自我效能水平，以提高其生活质量。

Objective To explore the condition of self-efficiency and quality of life （QOL） in preoperative hospitalized patients with lumbar disc herniation （LDH）, and analyze their relationship. Methods Totally 138 preoperative hospitalized patients with LDH in 4 tertiary hospitals in Shanghai were investigated by general information questionnaire, QOL questionnaire （SF-36） and the general self-efficiency questionnaire, and the relation between them was analyzed. Results The score of self-efficiency of preoperative hospitalized patients with LDH was （24.15±6.57）, which was lower than the norm （t=-7.671 ,P〈0.01）. The QOL score was （ 53.71 ± 19.38 ）, and there were significant differences in QOL score among patients with different age, occupation, working position, lumbar load and education level （ F = 4.119,6.207,5.972,5.064, 4. 703; P 〈 0. 01 ）. The self-efficiency of patients was positively correlated with the physilogical function, physilogical role, body pain, total health codition, social function and emotional function in QOL （r = 0.186- 0. 389;P〈0.05）. Conclusions The self-efficiency and QOL of patients with lumbar disc hemiationare all lower than the norm, and nurses should take effective nursing measures to improve the self-efficiency level of patients with lumbar disc herniation, so as to improve their QOL.
