输血相关急性肺损伤大鼠肺组织细胞间黏附分子1和细胞因子诱导的中性粒细胞趋化因子1的表达

Expressions of intercellular adhesion molecule-1 and cytokine-induced neutrophil chemoattractant-1 in rats with transfusion-related acute lung injury

摘要 目的检测输血相关急性肺损伤(TRALI)大鼠肺组织细胞间黏附分子1(ICAM-1)和细胞因子诱导的中性粒细胞趋化因子1(CINC-1)的表达,探讨TRALI的发病机制。方法雄性SD大鼠60只按随机数字表法分为正常对照组、脂多糖对照组、TRALI组和阳性对照组,每组15只。(1)TRALI组:腹腔注射脂多糖2 h后进行大鼠股动脉插管,移除大鼠约10%血容量的血液,然后输注等体积的血浆;(2)正常对照组:除腹腔注射生理盐水2 ml,以及移除大鼠血液后输注等体积生理盐水外,其余处理同TRALI组;(3)脂多糖对照组:移除大鼠血液后输注等体积生理盐水,其余处理同TRALI组;(4)阳性对照组:移除大鼠血液后输注等体积脂多糖,其余处理同TRALI组。4组大鼠处理6 h后处死,进行肺组织病理学检查和髓过氧化物酶(MPO)活性测定,检测支气管肺泡灌洗液细胞总数和中性粒细胞百分比。采用反转录-聚合酶链反应(RT-PCR)和免疫组化法检测4组大鼠肺组织ICAM-1、CINC-1 mRNA和蛋白表达。结果 TRALI组大鼠肺组织病理评分、湿干重比和MPO活性均明显高于正常对照组(均P〈0.05)。与正常对照组比较,TRALI组大鼠支气管肺泡灌洗液细胞总数和中性粒细胞百分比均明显升高(均P〈0.05)。TRALI组大鼠肺组织ICAM-1、CINC-1 mRNA表达均明显高于正常对照组(均P〈0.05)。免疫组化显示,TRALI组大鼠肺组织ICAM-1、CINC-1蛋白表达均明显高于正常对照组。结论 ICAM-1和CINC-1参与了TRALI模型大鼠肺组织中性粒细胞与内皮细胞的黏附和聚集,可能在TRALI发病过程中起重要作用。 Objective To determine the expressions of intercellular adhesion molecule-1(ICAM-1)and cytokine-induced neutrophil chemoattractant-1(CINC-1)in rats with transfusion-related acute lung injury(TRALI),and to investigate the pathogenesis of TRALI. Methods Sixty male SD rats were randomly divided into the normal control group,lipopolysaccharide control group,TRALI group and positive control group(n=15 each). (1)TRALI group:At 2 h after intraperitoneal injection of lipopolysaccharide,femoral artery was cannulated. About 10%of the blood volume was removed from the rats,and then an equal volume of plasma was infused.(2)Normal control group:Except for intraperitoneal injection of 2 ml normal saline, and the infusion of the same volume of normal saline after the removal of rat blood,the remaining treatments were the same as those in the TRALI group.(3)Lipopolysaccharide control group:Except for infusion of the same volume of normal saline after the removal of rat blood,the remaining treatments were the same as those in the TRALI group.(4)Positive control group:Except for infusion of the same volume of lipopolysaccharide after the removal of rat blood,the remaining treatments were the same as those in the TRALI group. The rats in the 4 groups were sacrificed at 6 h after the treatment,and subjected to lung histopathological examination and myeloperoxidase(MPO)activity determination. The total number of cells and percentage of neutrophils in the bronchoalveolar lavage fluid were measured. The mRNA and protein expressions of ICAM-1 and CINC-1 in the rat lung tissues of 4 groups were determined by reverse transcription-polymerase chain reaction(RT-PCR) and immunohistochemistry. Results The lung histopathological score,wet-dry weight ratio and MPO activity in the TRALI group were significantly higher than those in the normal control group (all P〈0.05). Compared with the normal control group, the total cell count and neutrophil percentage of bronchoalveolar lavage fluid in the TRALI group were significantly increased(both P〈0.05). The expression of ICAM-1 and CINC-1 mRNA in lung tissue of TRALI group was significantly higher than that of the normal control group(both P〈0.05). Immunohistochemistry showed that the ICAM-1 and CINC-1 protein expression in the lung tissues of the TRALI group was significantly higher than that of the normal control group. Conclusion ICAM-1 and CINC-1 involve in the adhesion and aggregation of neutrophils and endothelial cells in the lung tissues of TRALI model rats,which may play an important role in the pathogenesis of TRALI.
