进展期胃癌应用SOX方案新辅助化疗对手术相关并发症的影响探讨

Investigation of influence by SOX regimen for neoadjuvant chemotherapy applied in advanced stomach cancer on operation-related complications

目的探讨胃癌进展期采用SOX方案辅助化疗对手术并发症产生的影响。方法 40例进展期胃癌患者作为研究对象,随机分为研究组和对比组,各20例。研究组采用SOX方案(替吉奥胶囊+奥沙利铂)辅助化疗,对比组采用EOF方案(表柔比星+奥沙利铂+氟尿嘧啶)辅助化疗。观察两组患者并发症发生情况。结果研究组术后出血、术后伤口感染裂开、粘连性肠梗阻、吻合口瘘发生例数分别为4、3、2、1例,而对比组分别为6、5、4、2例,研究组并发症发生率为50.0%,明显低于对比组的85.0%,差异具有统计学意义(P<0.05)。结论 SOX方案有助于提高进展期胃癌患者耐受性,降低术后并发症发生率,在进展期胃癌患者接受手术治疗前,应当提倡采用SOX方案治疗。

Objective To investigate influence by SOX regimen for adjuvant chemotherapy applied in advanced stomach cancer on operation complications. Methods A total of 40 patients with advanced stomach cancer as study subjects were randomly divided into research group and comparison group, with 20 cases in each group. The research group received SOX regimen（tegafur gimeracil oteracil potassium capsules ＋ oxaliplatin） for adjuvant chemotherapy, and the comparison group received EOF regimen（pharmorubicin ＋ oxaliplatin ＋ fluorouracil） for adjuvant chemotherapy. Observation was made on complications condition in both groups. Results The research group had 4 cases with postoperative hemorrhage, 3 cases with postoperative incision infection and rupture, 2 cases with adhesive intestinal obstruction and 1 case with anastomotic fistula, while the comparison group had respectively 6, 5, 4 and 2 cases. The research group had obviously lower incidence of complications as 50.0% than 85.0% in the comparison group, and their difference had statistical significance（P〈0.05）. Conclusion SOX regimen is helpful to improve tolerability by advanced stomach cancer patients and reduce incidence of postoperative complications. Implement of SOX regiment for chemotherapy is recommended before operation treatment for patients with advanced stomach cancer.
