异体输血对围手术期胃肠道恶性肿瘤患者免疫功能及生活质量的影响

Influence of allogeneic transfusion on immune function and life quality of patients with gastrointestinal malignant tumors during perioperative period

目的探讨异体输血对围手术期胃肠道恶性肿瘤患者免疫功能及生活质量的影响。方法 30例胃肠道恶性肿瘤患者,随机分为A组和B组,每组15例。A组患者围手术期给予异体输血治疗,B组患者不给特殊处理,分析比较两组患者手术后的免疫功能变化和生活质量变化。结果 A、B两组患者术前1 d、术后1d以及术后7 d的白细胞介素-2(IL-2)水平和前列腺素E2(PGE2)水平比较差异均无统计学意义(P>0.05)。A组患者中功能状态评分标准(KPS)评分提高>10分者12例,评分无明显变化者2例,评分下降者1例;B组患者中提高>10分者6例,评分无明显变化者5例,评分下降者4例。A、B两组患者的KPS评分>10分的例数比较差异具有统计学意义(P<0.05)。结论胃肠道恶性肿瘤患者围手术期给予异体输血在术后短时间内不会对免疫功能造成明显影响,同时还有利于改善患者的生活质量。

Objective To explore influence of allogeneic transfusion on immune function and life quality of patients with gastrointestinal malignant tumors during perioperative period. Methods A total of 30 patients with gastrointestinal malignant tumors were randomly divided into group A and group B, with 15 cases in each group. The group A received allogeneic transfusion during perioperative period, and the group B received no special treatment. Comparison were made on postoperative immune function and life quality of two groups. Results Both groups had no statistically significant difference in interleukin-2（IL-2） and prostaglandin E2（PGE2） of 1 day preoperation, 1 day and 7 days postoperation（P〉0.05）. Group A had 12 increased 〉10 points cases in Karnofsky performance status（KPS） score, 2 cases with no obvious change and 1 case with decreased points. Group B had 6 increased 10 points cases,4 cases with no obvious change and 5 cases with decreased points. Difference had statistical significance in cases of KPS score increased 〉10 points in two groups（P〈0.05）. Conclusion Allogeneic transfusion shows no clear influence on immune function of patients with gastrointestinal malignant tumors during perioperative period and contribute to improve life quality of patients.
