Mulligan松动术联合Mckenzie疗法治疗神经根型颈椎病43例疗效观察

Mulligan Manipulation Combined with Mckenzie Mechanics Method in Treating 43 Cases of Cervical Spondylotic Radiculopathy

目的:观察Mulligan松动术联合Mckenzie疗法治疗神经根型颈椎病的临床疗效。方法:86例患者按照随机数字表分为治疗组和对照组,每组43例,2组均给予颈椎牵引及超短波基础物理治疗,治疗组加以Mulligan松动术及Mckenzie功能训练,对照组配合传统中医推拿手法。每天1次,10次为1个疗程。治疗前后用视觉模拟评分法(VAS)、颈椎病临床评价量表(CASCS)、SF-36生存质量量表(SF-36)、治愈显效率及总有效率对2组疗效进行评价。结果:2组治疗后VAS评分均较治疗前降低(P<0.01),且治疗组的降低幅度大于对照组(P<0.05);2组治疗后CASCS、SF-36评分均较治疗前升高(P<0.01),且治疗组的升高幅度明显大于对照组(P<0.01或P<0.05);2组治疗后临床治愈显效率及总有效率比较,差异有统计学意义(P<0.01)。结论:在颈椎牵引及超短波基础物理治疗基础上,加Mulligan松动术联合Mckenzie疗法能显著改善神经根型颈椎病患者的症状、体征及功能状态,有效提高生存质量,康复效果良好。

Objectlve：To observe the curative effects of Mulligan manipulation combined with Mckenzie mechanics method for the treatment of cervical spondylotic radiculopathy （CSR）. Methods ： A total of 86 CSR patients were assigned to the treatment group and the control group by a random number table, and 43 patients in each group. Two groups received the basic physical thera- py, such as cervical traction and ultrashort wave therapy. The treatment group was added with Mulligan manipulation and Mckenzie ffmction training, and the control group was added with traditional Chinese medicine manipulation. One time per day and every ten times was a course of treatment. Some measurements including pain visual analog scale （VAS）, clinical assessment scale for cervical spondylosis （CASCS）, the MOS Item Short From Health Survey （SF-36） were recorded to evaluate the curative effects before and after the treatment. Results：The VAS scores in the two groups were decreased after the treatment （P〈0.01）, and the decreasing amplitude of treatment group was higher significantly than that of the control group （P〈 0.05 ） ;the scores of CASCS and SF-36 in the two groups were increased after the treatment （P〈 0.01 ）, and the increasing amplitude of treatment group was higher significantly than that of the control group （P〈 0.05 ）;there were significant differences in cure rate and total effective rate between the two groups （P〈 0.01 ）. Conclusion：On the basis of cervical traction and ultrashort wave therapy, Mulligan manipulation combined with Mckenzie function training has positive effects on the improvement of the symptoms, signs and functional status and the quality of life for CSR patients.
