建立公共脐血库的策略

Strategies for establishing a public cord blood bank

脐血作为干细胞的来源,不仅被广泛用于造血干细胞移植(HSCT),而且被用于临床细胞治疗以及再生医学研究。全球范围已建立了数以百计的公共或私人脐血库,并成立了国际脐血制品登记处(BMDW)。虽然公共脐血库提供几乎所有脐血用于HSCT的脐血,但是其中大多数面临财务困境。本文介绍各种类型脐血库的特点,影响脐血制品使用率的因素,讨论提高公共脐血使用率及维持财务平衡的若干问题,包括选择性采集高质量脐血,设置最佳库容量,脐血种族背景多样性,以及脐血的HLA分型和非遗传性母源抗原(NIMA)配型。

Umbilical cord blood （ UCB）, as an important source of stem cells, not only widely used in allogenic hemato- poietic stem cell transplantation （ HSCT）, but also used for clinical cell therapy and regenerative medicine. So far hundreds of public and private cord blood banks as well as the international public cord blood registries have been established world- wide. Although almost all cord blood units used for HSCT were provided by the public cord blood bank, but most of them were faced difficulties to maintain their financial sustainability.This review describes the characteristics of various types of cord blood bank and the factors that influence the utilization of cord blood units （CBU）. Several issues for increasing CBU u- tilization and improvement of financial sustainability were discussed, including selective collection of high-quality cord blood, the optimal size of UCB bank, ethnic background diversity of CBU, as well as HLA typing and Non-inherited maternal anti- gen（NIMA） matching.
