摘要
目的利用红细胞基因分型辅助鉴定对抗高频抗原的抗体(Antibody against high prevalence antigens)。方法对1例对抗高频抗原的抗体用血清学定型和基因定型(genotype),同时采用菠萝酶与巯基试剂(DTT)处理红细胞进行抗体鉴定。结果抗筛细胞与谱细胞反应均阳性、反应强度一致且直抗和自身对照阴性,患者血清中存在对抗高频抗原的抗体。用试剂抗体血清学方法未检出稀有血型抗原,而基因分型检出患者为稀有血型Di(a+b-),其血清中存在的高频抗原抗体高度怀疑为抗-Di^b。结论红细胞基因定型在辅助鉴定对抗高频抗原的抗体中有重要意义。
Objective To identify antibody against highly prevalent antigens through RBC genotyping. Methods Phe- notyping and genotyping were conducted on a case with antibody against highly prevalent antigens. Antibody identification was carried out using Bromelin and DTT to treat RBC. Results Screen cell and panel cell showed positive results without obvious difference in titers, while the DAT and auto-control showed negative reshs. These results indicated the existence of antibody against highly prevalent antigens. No rare blood group was identified using Commercial monoclonal antibody, while the rare blood group of Di (a+b-) in this case was identified using RBC genotyping. The antibody against highly prevalent antigens in this case was anti-Dib. Conclusion RBC genotyping is a potential and powerful tool for identification of antibody against highly prevalent antigens.
