抗-Mur引起的交叉配血不合2例并文献复习

Incompatible crossmatch due to anti-Mur:two case reports and literature review

目的通过对抗-Mur引起的2例抗体筛查阴性,交叉配血不合病例的抗体分析及鉴定,了解不同抗原谱抗筛细胞对抗体筛查实验结果的影响,提高对Mur抗原、抗体的临床认识。方法利用3家厂商的抗体筛查细胞对2例抗体筛查实验阴性但交叉配血不合的病例进行抗体检测、鉴定,并对相关文献进行复习。结果 2例病例除与1套含"Mia"抗原的抗筛细胞出现阳性反应,与其余抗筛细胞反应均为阴性。抗体鉴定结果为抗Mur。2例自身红细胞与抗-Mur血清凝集反应阴性,但与之配血不相合的献血者血样与抗-Mur血清反应阳性。结论导致2例抗体筛查阴性而交叉配血不合的原因为抗Mur,商品抗筛细胞应包含在中国人群中分布频率较高的Mur抗原。

Objective To understand the effect of different antibody screening cell reagent on antibody screening results and to improve the knowledge of Mur antigen and anti-Mur. Methods Two cases of incompatible crossmatches due to anti- Mur were detected by different antibody screening cell reagents from three manufacturers and were verified by antibody detec- tion reagent red cells. Meanwhile, the related literatures were reviewed. Results The serum of two patients was detected by three types of screening ceils. Positive results were observed with the one that had the ＂Mia＂ antigen. Anti-Mur was detected by antibody detection testing. The red blood cells of two patients were not reactive with anti-Mur, whereas the incompatible donors＂ blood mixed with patients＂ showed a positive rusult for anti-Mur. Conclusion Mur antigen should be included on the antibody screening cell reagent.
