降钙素原在儿童下呼吸道感染治疗中的应用研究

Application research of procalcitonin in treatment of lower respiratory tract infection in children

目的分析及探索血清降钙素原在下呼吸道感染患儿治疗中的临床应用价值。方法收集本科室2013年10月-2015年10月间接收的患下呼吸道感染的100例患儿,将患儿随机分成两组。参照组共有50例患儿,给予常规抗生素治疗方案进行治疗;观察组共有50例患儿,先对患儿进行血清降钙素原检查,再根据检查结果采用抗生素进行治疗。治疗完毕后对两组患儿的临床疗效进行评估比较分析,详细记录两组患儿用药后的症状及体征消失时间,抗生素用药时间、抗生素花费及住院天数。结果与参照组患儿相比,观察组患儿治疗后的总有效率显著更高,抗生素用药时间、住院天数显著更短,抗生素花费显著更少（P〈0.05）。两组患儿治疗后的发热消失时间与咳嗽消失时间对比差异无统计学意义（P〉0.05）。结论血清降钙素原指导下用药能够有效提高下呼吸道感染患儿的疗效,且有效减少其抗生素用药及住院时间,值得广泛推广。

Objective To analyze and explore the clinical value of serum procalcitonin in treatment of lower respiratory tract infection in children. Methods 100 cases of children with lower respiratory tract infection cured in the department from October 2013 to October 2015 were selected as the study objencts. And they were divided into two groups randomly. The reference group had 50 cases, they were treated with outine antibiotic regimens for treatment, and the observation group had 50 cases, they were treated with serum procalcitonin first, and then antibiotic treatment according to test results. After the treatment, the clinical effect of the two groups were accessed,compared and analyzed. The symptoms and signs disappeared time, antibiotics time, antibiotics costs and hospitalization days of the two groups were detailly recorded after the treatment. Results Compared with the reference group, the clinical efficiency of the observation group was significantly higher. Antibiotics time, days of hospitalization were significantly shorter. Antibiotics costs was significantly less（P〈0.05）. After the treatment, fever disappearance time and cough disappearance time of the two groups had no significantly different（P〉0.05）. Conclusion Serum procalcitonin guidance of medication can improve the efficacy of lower respiratory tract infection in children, and reduce antibiotic treatment and hospitalization time. Iit should be popularized.
