神经内镜技术与立体定向技术应用于高血压性脑出血的临床疗效

Comparison of the application of neural endoscopy and stereotactic technique in hypertensive cerebral hemorrhage

目的对比研究神经内镜技术与立体定向技术应用于高血压性脑出血（HICH）的临床治疗效果。方法选取2013年6月～2015年8月，我院神经外科接受神经外科神经内镜血肿清除术治疗的30例HICH患者作为观察组，同期接受神经内科立体定向钻孔血肿抽吸术治疗的30例HICH患者作为对照组，对比分析两组的临床疗效。结果观察组的手术时间与术中失血量明显多于对照组（P〈0．05）；观察组的血肿清除率显著高于对照组（P〈0．05）；术后6个月观察组的GOS评分及预后分级均显著优于对照组（P〈0．05）；术后观察组的GCS评分及BI指数均显著高于对照组（P〈0．05）；观察组术后并发症率为13．33％，显著低于对照组的23．33％（P〈0．05）。结论相比于立体定向钻孔血肿抽吸术，神经内镜下血肿清除术的近远期疗效更为显著，值得推广应用。

Objective To compare the clinical efficacy of neuroendoscope as opposed to ste reotactic techniques in hypertensive intracerebral hemorrhage （HICH）. Methods From June 2013 -2015 year in August,the Department of Neurosurgery of our hospital department of Neu- rosurgery received endoscopic hematoma in 30 cases of HICH patients were treated as the obser- vation group, 30 cases of HICH patients underwent neurological stereotactic drilling hematoma aspiration in the patients as the control group,compared the clinical efficacy of the 2 groups. Re- sults The operative time and blood loss in the observation group were obviously higher than the control group （P〈0.05） ; the clearance rate of hematoma in observation group was significantly higher than the control group （P〈0.05） ; the GOS score and recovered 6 months of observation group were significantly better than the control group （P〈0.05）; The observation group the GCS score and BI index after operation were significantly higher than control group （P〈0.05） ; postoperative complications in the observation group was 13. 33%, significantly lower than 23. 33% in the control group （P〈 0.05）. Conclusion Compared with stereotactie drilling hematoma aspiration the curative effect of endoscopic hematoma is more significant and is worthy of popular ization and application.
