摘要
目的:探讨慢性阻塞性肺疾病急性加重期(AECOPD)患者的肿瘤坏死因子(TNF-α)和可溶性肿瘤坏死因子s TNF-R55、s TNF-75的水平变化。方法:选择2015年8月-2016年8月在本院就诊的120例AECOPD患者作为观察组,并选择同时期的120健康志愿者作为对照组,对比两组患者在入院第1、7天的TNF-α和s TNF-R55、s TNF-75水平三项指标以及肺功能。结果:观察组治疗后其肺功能有了明显改善,但与对照组相比差异有统计学意义(P<0.05)。观察组治疗前后血浆TNF-α和s TNF-R55、s TNF-75水平相比于对照组有明显的提高,差异均有统计学意义(P<0.05);此外在对照组中吸烟者的TNF-α也是要明显高于不吸烟者(P<0.05)。观察组在第7天血浆中的TNF-α水平要低于入院第1天(P<0.05),而s TNF-R55、s TNF-75水平较前有一定程度上升,但差异均无统计学意义(P>0.05)。结论:对于AECOPD患者而言,其血中的TNF-α和s TNF-R55、s TNF-75水平相比于正常人群要明显高出,通过积极正规的治疗后,TNF-α水平有明显下降,但是抗炎介质水平上升程度有限。
Objective:To observe the clinical expression of TNF-α,s TNF-R55 and s TNF-75 for acute exacerbation of chronic obstructive pulmonary disease(AECOPD) patients.Method:A total of 120 AECOPDpatients treated in our hospital from August 2015 to August 2016 were selected as the observation group.120 healthy volunteers were selected as the control group.After 1,7 d of admission,the levels of TNF-α,s TNF-R55 and s TNF-75 and the pulmonary function of two groups were compared.Result:After treatment,the pulmonary function for AECOPD patients was greatly improved,however the pulmonary function of the observation group and the control group was significantly different(P〈0.05).Before and after treatment,the levels of TNF-α,s TNF-R55 and s TNF-75 of the observation group were significantly higher than those of the control group(P〈0.05).In the control group,the level of TNF-α of smoking patients was higher than that of non-smoking patients(P〈0.05).In the observation group,the level of TNF-α after 7 d of admission was lower than that of after 1 d of admission(P〈0.05).After treatment,the levels of s TNF-R55 and s TNF-75 were slightly better than those of before treatment,but there were no statistical significance(P〉0.05).Conclusion:AECOPD patients,the levels of TNF-α,s TNF-R55 and s TNF-75 are higher than those of healthy volunteers.Through the positive and regular treatment,the level of TNF-α is greatly reduced,however,the anti-inflammatory mediators are increased in a lower way.
