电针不同波形、频率搭配组合方案治疗急性脑梗死临床观察

Clinical Observation of Electro-acupuncture with Different Waveform and Frequency Collocation Combination Therapy for Acute Cerebral Infarction

目的:观察电针不同波形、频率搭配组合方案治疗急性脑梗死的临床疗效。方法:将90例脑梗死急性期患者按随机数字表分为9组,研究电针波形(A:连续波,断续波,疏密波),频率(B:连续波、断续波2Hz、50Hz、100Hz;疏密波2/10Hz、10/50Hz、20/100Hz)2因素3水平9组不同搭配组合方案,14d为1疗程,连续治疗2个疗程。对治疗前后NIHSS评分,ADL评分及简化Fugl-Meyer评分进行分析并比较其疗效。结果:4周后各组患者NIHSS评分、ADL评分及Fugl-Meyer评分均明显改善;不同波形的电针在改善NIHSS评分,ADL评分及Fugl-Meyer评分方面无差异(P>0.05);电针频率是改善NIHSS评分的主效应,且100Hz频率电针在改善NIHSS评分方面更优(P<0.05)。结论:在头针基础上联合不同波形、频率电针治疗急性脑梗死是安全并且疗效确切,在临床上可轮替使用连续波、疏密波、断续波这3种波形,频率可选用对神经功能缺损评分改善明显的100Hz。

Objective： To observe the clinical effects of different waveforms and frequency combinations of electro- acupuncture on acute cerebral infarction. Methods： 90 patients with acute cerebral infarction were divided into 9 groups according to the random number table. The waveforms of electro-acupuncture （A： continuous wave, intermittent wave, light density wave）, frequency （B： continuous wave, 50Hz, 100Hz; 2 / 10Hz, 10 / 50Hz, 20 / 100Hz） 2 factors 3 levels of 9 groups with different combinations of programs, 14d for 1 course of continuous treatment of 2 courses. NIHSS score, ADL score and simplified Fugl-Meyer score before and after treatment were analyzed and compared. Results： NIHSS score, ADL score and Fugl-Meyer score were significantly improved in all groups after 4 weeks. There was no signifi- cant difference in NIHSS score, ADL score and Fugl-Meyer score （P〉0.05） Frequency was the main effect of improving NIHSS score, and 100Hz frequency electro-acupuncture was better in improving NIHSS score （P〈0.05）. Conclusion： Combining different waveforms and frequency electro-acupuncture in the treatment of acute cerebral infarction on the basis of scalp acupuncture is safe and effective. In clinical practice, three waveforms of continuous wave, dense wave and intermittent wave can be used alternately. Neurological deficits improved markedly by 100 Hz.
