不同途径使用氨甲环酸降低全髋关节置换术围术期失血的有效性和安全性 被引量:2

The efficacy and safety of different administration ways of tranexamic acid in reducing blood loss during the perioperative period of total hip replacement

摘要 目的探究不同途径使用氨甲环酸降低全髋关节置换术(THA)围术期失血的有效性和安全性。方法将近年收治的120例需THA手术患者,根据氨甲环酸不同给药途径,分为A组、B组、C组各40例,其中A组行单一静脉滴注,B组行单一局部灌注,C组行静脉联合局部灌注氨甲环酸,观察对比三组控制围术期失血的有效性和安全性。结果C组术后24h引流量、围术期总失血量低于A、B两组,术后第3dHb、Hot水平高于A、B两组,差异有统计学意义(P〈0.05),A、B两组之间各指标比较无统计学差异(P〉0.05)。A、B、C三组输血率分别为10.0%(4/40)、12.5%(5/40)、2.5%(1/40),C组输血率明显低于A、B两组(P〈0.05),A、B两组输血率比较无统计学差异(P〉0.05)。三组并发症率分别为7.5%、5.0%、2.5%,差异无统计学意义(P〉0.05)。结论静脉联合局部灌注在控制围术期总失血量方面效果更为显著,可降低输血率,且安全性较高,不增加深静脉血栓和肺栓塞的风险。
