中医综合理疗对全胃切除术后患者肠道功能免疫功能及生活质量的影响

Effect of Traditional Chinese Medicine comprehensive physiotherapy on intestinal function,immune function and life quality in patients after total gastrectomy

目的探讨中医综合理疗技术对全胃切除术后患者肠道功能、免疫功能及生活质量的影响。方法选取2011年10月至2014年12月间首都医科大学附属北京世纪坛医院收治的112例实施全胃切除术患者,采用随机数字表法分为观察组与对照组,每组56例。观察组患者实施中医综合理疗技术,对照组患者实施常规护理,比较两组患者术后并发症、营养学指标、免疫学指标、肠道功能和生活质量。结果两组患者术后10d总蛋白、白蛋白、前白蛋白、转铁蛋白、CD4^+、CD8^+、CD4^+/CD8^+、NK细胞、IgM、IgG、生理功能评分、生理角色限制评分、躯体疼痛评分、总体健康评分、活力评分、情感角色限制评分、社会功能评分和心理健康评分比术后1d均有增加,差异均有统计学意义(均P<0.05),且观察组患者以上各项评分均高于对照组,差异均有统计学意义(均P<0.05)。观察组患者术后并发症发生率低于对照组,术后肠鸣音恢复时间、术后肛门排气时间、术后排便时间和恢复进食时间均早于对照组,术后住院时间少于对照组,两组比较,差异均有统计学意义(均P<0.05)。结论中医综合理疗技术有助于改善全胃切除术患者术后肠道功能与免疫功能,并提高生活质量。

Objective To investigate the effect of Traditional Chinese Medicine（TCM） comprehensive physiotherapy on intestinal function,immune function and life quality after total gastrectomy.Methods Select 112 patients undergoing total gastrectomy at Beijing Millennium Monument Hospital Affiliated to Capital University of Medical Sciences from October 2011 to December 2014.These patients were randomly divided into an observation group and a control group with 56 patients in each group.Patients in the observation group were treated with TCM physiotherapy and patients in the control group received routine treatment and nursing.Postoperative complications,nutrition index,immunology index,intestinal function and quality of life were compared between the two groups.Results Compared with postoperative 1 d,overall levels of protein,albumin,pre-albumin,transferrin,CD4^＋,CD8^＋,CD4^＋/CD8^＋,NK cells,IgM and IgG,and physiological function,physiological role restriction,somatic pain,overall health,activity,emotional role restriction,social function and mental health scores increased in both groups at postoperative 10 d（P〈0.05）.Levels of protein,albumin,pre-albumin,transferrin,CD4^＋,CD8^＋,CD4^＋/CD8^＋,NK cells,IgM,IgG,and physiological function,physiological role restriction,somatic pain,overall health,activity,emotional role restriction,social function and mental health scores were higher in observation group than in the control group（all P〈0.05）.Postoperative complication rate was lower in observation group than in the control group（all P〈0.05）.Time to postoperative recovery of bowel sounds,postoperative anal exhaust time,postoperative defecation time and eating recovery time was shorter in the observation group than in the control group（P〈0.05）.Postoperative hospitalization time was less in the observation group than in the control group（P〈0.05）.Conclusion TCM physiotherapy is helpful in improving intestinal function,immune function and quality of life in patients undergoing total gastrectomy.
