慢性病患者延续性护理的研究现状

The present research situation of transitional care for elders with chronic diseases

随着人口老龄化速度加快,慢性病人数大幅增加。慢性病病情复杂且为终身性疾病,延续护理对于慢性病管理有着不容忽视的作用。本文从延续护理的概念、护理模式、存在问题及建议等方面进行综述,以期为探讨符合我国国情的慢性病延续护理模式提供依据。

Along with the rapidly aging population, the prevalence of chronic diseases also increased greatly in the elderly. Chronic diseases are complex and lifelong conditions, and transitional care plays an important role in the chronic disease management. In this review, the authors summarized the concept of transitional care, care model, problems and suggestions for implementing transitional care in patients with chronic diseases.
