2008-2014年云南省基本医疗卫生资源区域配置差异性及包容性增长分析

Analysis of Basic Health Resources Arrangement and It's Inclusive Growth in Yunnan Provice from 2008 to 2014

目的:了解云南省各区域基本医疗卫生资源配置的差异状况及其主要原因,为卫生管理部门均衡配置卫生资源提供决策依据。方法:收集云南省2008-2014年云南省各地区的基本医疗卫生资源信息及人口信息,采用泰尔指数测算各区域卫生资源的差异性。结果:2008-2014年云南省基本医疗卫生资源逐年增加,尤其医院个数与床位数的增幅超过15%;历年各医疗卫生资源在区域间的泰尔指数均高于区域内(除2012年床位资源泰尔指数区域间比区域内小0.0002);云南省中部、东北部、西北部和南部地区基本医疗卫生资源总体差异呈现缩小趋势;医院及卫生技术人员配置的公平性显著低于医疗机构和床位分布的公平性。结论:云南省在医疗卫生机构及床位的配置上均衡性不断改善,但在医疗技术人员配置上还应重点解决区域间的差异从而改善医疗资源软件配置的公平性。

Objective： To understand the current different status and main reasons of basic health resources arrangement among different regions in Yunnan Province, and provide decision evidences about health resource balanced arrangement for related policy makers. Methods：Collected the health resource and population information of Yunnan Povince from 2008 to 2014, and the Theil Index Analysis was applied to explore the differences of health resources between regions in Yunnan Povince. Results： It was found that the general gap of health resource arrangement showed a decreasing trend. In particular, the number of hospitols and the num- ber of beds increased by more than 15%. Over the years, except for the bed source in 2012 （ Theil index between regions was 0.0002 smaller tham within the region） , the theil indexes of the medical resources between regious are higher than within the re- gion. The differeuce of basic medical and health services in the central, northeast, northwest and southern regions of yunnan province showed a trend of narrowing. The equality of hospital and health workers arrangement was worse than that of health institutes and hospital beds.Conclusion：The arrangement in health institutions and hospital beds was becoming better in Yunnan Province. However, more efforts should be put to the arrangement of health workersto reduce the difference between regions so as to improve the equity of medical resource arrangement.
