脑膜瘤中ADC值、rADC值与Ki-67增殖指数的相关性分析 被引量:5

The Analysis of the Relations of ADC and rADC with Ki-67 Proliferation Index in Meningioma

摘要 目的探讨脑膜瘤的表观扩散系数(ADC)值、相对表观扩散系数(rADC)值与Ki-67增殖指数的相关性,提高术前评估脑膜瘤复发风险的准确率。方法回顾性分析经病理证实且行Ki-67阳性率测定的47例脑膜瘤患者资料,术前均行常规MRI及DWI检查,测量肿瘤实质、肿瘤实质对侧镜面正常区的ADC及ADCwm值,计算rADC值(ADC/ADC_(wm))并分析ADC、rADC与Ki-67增殖指数的相关性。结果脑膜瘤的ADC值与Ki-67增殖指数呈负相关(r^2=0.33,P<0.001),rADC值与Ki-67增殖指数也呈负相关(r^2=0.31,P<0.001)。结论 ADC值、rADC值与Ki-67增殖指数的负相关关系对术前明确脑膜瘤的病理级别及手术方式的选择很有价值。 Objective To evaluate the relations of ADC and r ADC with Ki-67 proliferation index in meningioma and to improve the diagnosis accuracy of the risk of recurrence before operation. Methods MRI examinations and histopathology of 47 surgically treated meningiomas were retrospectively reviewed. Mean ADC values and ADCwm values were derived from diffusion imaging and r ADC values could be calculated. Correlation coefficients were calculated for mean ADC,r ADC and Ki-67 proliferation index values using linear regression. Results A statistically significant inverse correlation was found between ADC,r ADC and Ki-67 proliferation index for low-grade and aggressive meningiomas( r^2= 0. 33,P 〈0. 001; r^2=0. 31,P 〈0. 001). Conclusion ADC values and r ADC values correlate inversely with Ki-67 proliferation index. It can be used for histopathological characterization of the meningiomas and presurgical planning.
