摘要
目的观察基质金属蛋白酶-3(MMP-3)、金属蛋白酶组织抑制剂-1(TIMP-1)及人类软骨糖蛋白-39(HC gp-39)在早期类风湿关节炎(RA)患者血清中的变化及临床意义。方法 2014年2月至2015年6月在该院接受治疗的早期RA患者50例纳入早期RA组,同期40例健康成年人纳入对照组。观察2组血清MMP-3、TIMP-1及HC gp-39水平,比较早期RA组中不同关节损伤程度患者的MMP-3、TIMP-1及HC gp-39水平的差异,探讨MMP-3、TIMP-1及HC gp-39与关节肿胀指数、关节压痛指数,以及相关实验室指标的相关性。结果早期RA组患者血清MMP-3、TIMP-1及HC gp-39水平明显高于对照组,差异均有统计学意义(P=0.001);有X线损伤组的早期患者MMP-3、TIMP-1及HC gp-39水平较无X线损伤组高,差异有统计学意义(P=0.001、P=0.001、P=0.018);MMP-3、TIMP-1及HC gp-39水平与关节肿胀指数、关节压痛指数、C3、C4、ESR和CRP水平呈显著正相关,而与患者的年龄、性别无显著相关。结论 MMP-3、TIMP-1及HC gp-39水平在早期RA患者中较高,且其水平与患者的临床症状及实验室指标密切相关。
Objective To observe the changes of matrix metalloproteinase-3(MMP-3),metal protease inhibitor-1(TIMP-1)and human cartilage glycoprotein-39(HC gp-39)in serum of early rheumatoid arthritis(RA)patients.Methods A total of 50 cases of early RA patients treated in our hospital from February 2014 to June 2015 were recruited into early RA group,while 40 healthy adults were selected as control group.The levels of serum MMP-3,TIMP-1and HC gp-39 were observed in two groups,and the levels of MMP-3,TIMP-1and HC gp-39 were compared in RA patients with different degree of joint damage.To investigate the relevance between MMP-3,TIMP-1,HC gp-39 and duration of morning stiffness,joint swelling index,joint pain index and related laboratory index.Results The levels of MMP-3,TIMP-1and HC gp-39 in serum in the early RA group were significant higher than those in the control group,the differences were statistically significant(P=0.001).The MMP-3,TIMP-1and HC gp-39 levels in the early stage of X-ray damage group were higher than those in the non X-ray group,and the difference was statistical significant(P=0.001,P=0.001,P=0.018).There were positive correlations between the level of MMP-3,TIMP-1,HC gp-39 and joint swelling index,joint tenderness index,C3 level,C4level,ESR level,CRP level,but there were no correlations with age,gender.Conclusion The levels of MMP-3,TIMP-1and HC gp-39 are higher in the patients with early RA,and their levels are closely related to the clinical symptoms and laboratory indexes of the patients.
