自制戴指套皮肤拉钩在保乳手术中的应用

Application of Self-made Wear Finger Skin Retractor in Breast Conserving Surgery

目的探讨自制戴指套皮肤拉钩在保乳手术中的应用及效果观察。方法选取我院乳腺外科2014年7月至2015年1月期间行保乳手术的121例乳腺癌患者,随机分为研究组(戴指套拉钩,n=58)与对照组(常规拉钩,n=63),并比较两组疗效。结果研究组术中皮肤灼伤发生率、术后切口脂肪液化发生率均小于对照组,差异有统计学意义(P<0.05)。两组组术后皮下积液、切口感染、手术时间比较差异无统计学意义(P>0.05)。试验组的瘢痕VSS评分、POSAS评分、观测者总体评分和患者总体评分均小于对照组,差异有统计学意义(P<0.05)。结论采用戴指套皮肤拉钩行保乳手术具有简易实用,减少手术皮肤灼伤、减少术后并发症,降低瘢痕形成等优点,值得在临床中推广。

Objective To explore the application and effect of self-made wear finger skin retractor in breast conserving surgery. Methods 121 cases of breast cancer patients in department of breast surgery in our hospital from July 2014 to January 2015 for breast conserving surgery were selected and randomly divided into study group（ with fingerstall hook, n = 58 ）and control group（ conventional retractor, n = 63）. And the effects of two groups were compared. Results The incidence of skin burn in the study group and the incidence of postoperative incision fat liquefaction were both less than those in the control group. And the difference was statistically significant（p 〈 0.05 ）. There were no significant differences in the postoperative subcutaneous effusion, incision infection and operation time of the two groups（ P 〉 0. 05 ）. The scar VSS score, POSAS score, total score of observer and to- tal score of patients in the study group were less than those of the control group, and the difference was statistically significant（ P 〈 0. 05）. Conclusion Wearing finger skin hook in breast conserving surgery has simple and practical operation, reducing skin burns, reducing postoperative complications, reducing scar formation and other advantages, which is worthy of clinical promotion.
